A previously healthy 65-year-old woman was referred due to breast pain, the physical examination revealed that both breasts were hard on palpation. Mammography revealed multiplate and scattered rounded nodular lesions in both breasts. The ultrasound revealed a snow pattern in both breasts. The patient later recalled to have received an injection of liquid silicone 20 years ago. Siliconomas are the result of a granulomatous reaction to silicone, either due to leakage of implants or due to direct injection. We recommend that in these patients magnetic resonance images should be obtained every year to rule out carcinoma in-situ.

Introduction:In contrast to other countries in Latin America, Peru had been notoriously spared by the global dissemination of carbapenem-resistant Klebsiella pneumoniae (CR-Kp) until recently. Isolated cases of KPC-producing K. pneumoniae have been reported since 2013 in Lima. However, by 2017, rapid emergence of CR-Kp took place at a tertiary care hospital in Lima. Here, we provide a description of the molecular characteristics of the CR-Kp isolates recovered during this emergence. Methods:We conducted a retrospective study of the emerging cases of CR-Kp infections diagnosed at a tertiary hospital in Lima, from July 2015 to December 2017. Kp was isolated from various clinical samples and profiled using the VITEK system and standard biochemical tests. Whole genome sequencing was performed in an Illumina MiSeq instrument at 50-100x coverage, and bioinformatic analysis was conducted using ...

The aim of this study was to evaluate the relative expression of the cytokines involved in the immune response of Th1 (IL-2, IL-12, IFN-γ, TNF-α), Th2 (IL-4, IL-10 and TGF-β) and Th17 (IL-17) in 21 guinea pigs (Cavia porcellus) experimentally inoculated with a field isolate of Salmonella Typhimurium at a dose of 102 CFU/ml, intraperitoneally, and compared with a control group (inoculated with a thermally treated strain). Blood samples were taken on days 1, 3, 5, 7, 9, 15 and 30 post-inoculation (p.i.). Total RNA was extracted from lymphocyte cells and real-time RT-PCR was conducted using specific primers for each of the cytokines. The relative mRNA expressions were determined by the comparative method 2-ΔΔCt with respect to the calibrator (healthy guinea pig) and using GAPDH as a constitutive gene of reference as a normalizer. The expressions of the cytokine genes in the Treatment g...

Climate-related phenomena in Peru have been slowly but continuously changing in recent years beyond historical variability. These include sea surface temperature increases, irregular precipitation patterns and reduction of glacier-covered areas. In addition, climate scenarios show amplification in rainfall variability related to the warmer conditions associated with El Niño events. Extreme weather can affect human health, increase shocks and stresses to the health systems, and cause large economic losses. In this article, we study the characteristics of El Niño events in Peru, its health and economic impacts and we discuss government preparedness for this kind of event, identify gaps in response, and provide evidence to inform adequate planning for future events and mitigating impacts on highly vulnerable regions and populations. This is the first case study to review the impact of a C...

Rickettsiae are a family of ectoparasite-borne bacteria that can produce high morbidity and mortality among humans. There are scarce data on rickettsial ecology in rural areas of the Peruvian Amazon basin, where seroprevalence has not been determined, and the identities of animals acting as reservoirs of these bacteria are not known. We conducted a cross-sectional study in Zungarococha (between 2019 and 2021), a rural community located approximately 20 km away from Iquitos city. Blood samples were collected from humans (175), dogs (123), and cats (12). Blood samples and tissues were collected from black rats (84). Finally, we collected fleas from dogs and cats (222), ticks from dogs (91), and mites from black rats (32). Blood samples from humans, dogs, cats, and black rats were analyzed by indirect immunofluorescence assays (IFA) to detect IgG antibodies against rickettsias. We screened ...

Objectives: Antimicrobial resistant (AMR) Campylobacter is a global health threat; however, there is limited information on genomic determinants of resistance in low- and middle-income countries. We evaluated genomic determinants of AMR using a collection of whole genome sequenced Campylobacter jejuni and C. coli isolates from Iquitos, Peru. Methods: Campylobacter isolates from two paediatric cohort studies enriched with isolates that demonstrated resistance to ciprofloxacin and azithromycin were sequenced and mined for AMR determinants. Results: The gyrA mutation leading to the Thr86Ile amino acid change was the only gyrA mutation associated with fluoroquinolone resistance identified. The A2075G mutation in 23S rRNA was present, but three other 23S rRNA mutations previously associated with macrolide resistance were not identified. A resistant-enhancing variant of the cmeABC efflux pump ...
