Up to 80% of patients with hypothyroidism present with neuromuscular symptoms. We report the case of a 56-year-old male with a two-year history of symptoms suggestive of hypothyroidism, which worsened over the last five months, with difficulty walking, muscle rigidity, and cramps. Laboratory tests confirmed primary hypothyroidism with elevated creatine kinase (CK) and lactate dehydrogenase (LDH) levels. Following treatment with levothyroxine, the clinical and biochemical response was favorable. Hoffmann syndrome, an atypical form of hypothyroid myopathy, is rare and primarily affects males with long-standing hypothyroidism. It is characterized by pseudohypertrophy and progressive muscle weakness, affecting less than 10% of hypothyroid patients. The pathogenesis is thought to involve alterations in the IGF1–PI3K–Akt/mTOR and myostatin-Smad3 pathways. Treatment with levothyroxine norma...

Postoperative pancreatic fistula (POPF) is the most common complication after cephalic pancreatoduodenectomy (CPD) and occurs in 3 to 45% of cases. It is associated with morbid sequelae including intra-abdominal sepsis, hemorrhage, among other. Which carries a high mortality up to 40%. Surgery is the best option in severe cases. Different surgical procedures have been tried for its treatment with different results. We present the case of a 60-year-old male patient who presented POPF after CPD and a wirsungostomy was performed as a bridge surgery until a pancreatogastro anastomosis was performed as definitive treatment.

We present the case of a 5-year-old male patient with a medical history of recurrent bronchospasms since 3 months . He went to the pediatric allergy clinic at the Edgardo Rebagliati Martins National Hospital with his mother, who reported episodes of acute urticaria, one after consuming cooked pork and another 2 after consuming cooked beef. To the evaluation, non-contributory exam. The following tests were performed: specific IgE in vitro, where a positive result was obtained for pork albumin (26.6 KUA/L) and later skin tests, with a positive result for raw pork. Due to the appearance of lesions only after the consumption of cooked mammalian meat, alpha-gal syndrome was suspected and specific IgE testing against alpha-gal was requested through an external laboratory, which came back positive (>100 kU/L).

Objective: To determine the association between the number of prenatal controls (PCN) and the prevalence of low birth weight (LBW) in individuals born during the post-COVID-19 era in Peru. Materials and Methods: This is a secondary study based on data from the 2022 National Demographic and Family Health Survey (ENDES). Low birth weight (LBW) was defined as a birth weight less than 2500 g. Sociodemographic and clinical variables were also evaluated to control for their influence and possible confounding effects on the study relationship. Results A prevalence of LBW of 4.1% was found. The adjusted analysis showed that for each completed CPN in pregnant women, the prevalence of LBW decreased by 12% (aPR: 0.88; 95% CI: 0.80-0.96). A significant association was also found between LBW and factors such as iron supplementation (aPR: 7.42; 95% CI: 1.05-52.32) and having Spanish as the mother tong...

Introduction: The infiltration of botulinum toxin type A causes chemical denervation of the abdominal wall muscles surrounding the hernia defects of complex eventrations, favoring their repair. Report: 11 cases aren presented with complex eventrations, whose lateral muscle complexes were infiltrated, with botulinum toxin type A (Botox®), as a prehabilitative measure. In the post-infiltration tomographic images, we observed a decrease in the transverse diameter of the right lateral complex between 0.2 to 2 cm and between 0.6 to 1.5 cm on the left side, which represented a reduction of 38% and 40% respectively. In more than 90% of cases, complete fascial closure of the defect was achieved. Conclusion: The use of botulinum toxin type A, in complex incisional hernias, due to its relaxing action on the infiltrated muscles, causes thinning, therefore muscle elongation, favoring aponeurotic cl...

Introduction: The present study arises in response to the sustained increase in dengue outbreaks in Latin America, with special emphasis on the state of Zulia, Venezuela. This region, composed of 21 municipalities, is highly vulnerable to dengue transmission. Given this scenario, it is essential to have tools that allow early detection of outbreaks and, thus, optimize prevention and public health intervention strategies. The main objective is to develop an early warning system for dengue outbreaks using machine learning (ML) techniques. Materials and methods: Several data sources are integrated: epidemiological information, meteorological parameters, El Niño and La Niña (Niño 3.4 Index), socioeconomic and demographic variables. Two ML models were used: Support Vector Machine for regression (SVM-R) and Gaussian Process Regression (GPR). Results: The predictions obtained showed remarkab...
