We present the case of a 44-year-old male patient with a family history of lymphoma and hyperthyroidism. He started the disease three months before admission to hospital with pain in the right leg, abdominal pain, weight loss and night sweats. He was diagnosed in his hospitalization with ascites in a cavity of undetermined etiology, and multiple polygastric and fossa lymph nodes right iliac. At 6 months, the symptoms persist and a nodule is added on the penis, left testicular edema and purulent discharge from the right inguinal node, being diagnosed orchiepididymitis. Initially he had negative markers for tuberculosis (TB), however, a lymph node biopsy was performed and the analysis with Xpert MTB/RIF where it resulted positive. The patient improved considerably with anti-TB treatment and antibiotics for purulent discharge.

This is the case of a 33 years old man with history of homosexual relations and heavy alcohol drinking. Tobacco, marijuana, cocaine, basic paste and cocaine hydrochloride consumption since age 18. Because of human immunodeficiency virus infection he received irregular treatment (2006-2008) with highly active antiretroviral therapy (HAAT), regulated in the past 12 months. As the patient showed positive resistant acid-fast bacilli he received since June 2008 standard antituberculous treatment [2RHZE/4(HR)2)] (R: rifampicin; H: isoniazid; Z: pirazinamide; E: ethambutol) that was withdrawn 6 months later when the patient showed pulmonary negative acid fast bacilli. In March 2009 he felt intense lumbar pain, difficulty in walking, hypertrophy of cervical lymph nodes, cough and fever. Objetive: To describe a pediatric case with initial diagnosis of lymph node tuberculosis (TB) that became multidrug resistant miliar and meningeal tuberculosis (TB-MDR) due to treatment dropout twice. Case report: a 4-year-old boy with initial diagnosis of lymph node tuberculosis who had two episodes of dropout from the sensitive scheme treatment. Three months later, there was evidence of bilateral involvement suggestive of miliary TB. During hospitalization, he developed meningeal tuberculosis and hydrocephalus. Due to the history of treatment dropout, a sensitivity of gastric aspirate study was requested, identifying it as multidrug-resistant TB. Tuberculosis affects one-third of the global population, with the highest incidence concentrated in developing countries and among immunosuppressed patients. Among its extrapulmonary manifestations, intestinal involvement ranks sixth in frequency, with a predilection for the ileocecal region, typically presenting as intestinal inflammatory changes and necrotic lymph nodes. Tuberculosis and brucellosis remain important causes of morbidity and mortality in many countries, for the detection of both diseases requires efficient and sensitive tool for effectuate the diagnosis. This study was aimed to evaluate and compare the duplex PCR versus the nested PCR, for detection of Brucella spp. (BR) and Mycobacterium spp. (TB). A total of 100 samples of tissues from tracheo-bronchial lymph nodes, bovine lung and bacterial isolate as positive controls were used. Were evaluated ten combinations of primers which were designed to flank the segment of the 16S rRNA sequence (RB) and antigen gen MPB70 (TB), the best result for the Duplex PCR was obtained with the primers Bru-2F/Bru-2R for BR and Tub-1F/Tub-N-R for TB.
