Tuberculosis affects one-third of the global population, with the highest incidence concentrated in developing countries and among immunosuppressed patients. Among its extrapulmonary manifestations, intestinal involvement ranks sixth in frequency, with a predilection for the ileocecal region, typically presenting as intestinal inflammatory changes and necrotic lymph nodes. The most common symptoms include abdominal pain, fever, and palpable mass, often associated with histological findings of confluent caseating granulomas and ulcerative mucosal lesions. Intestinal inflammatory processes can result from various etiologies. From an imaging perspective, it is essential to identify descriptors that help differentiate inflammatory-infectious conditions from neoplastic processes.
