Although inherent in medical task, in last century the patient care quality improvement procedures have achieved a significant development, becoming in cornerstone of an adequate diagnosis and treatment, which gave place to institutionalization of Medical Audit as a speciality within medical practice. Information presented in this review allows clinical history-based sytematic study of medical care process, its diagnosis and treartment guides-contrasted analysis detailinig the historical development, and the choice of unfolding at national context.

The aim of this paper is to show the development of the journal Annals of the Faculty of Medicine, the oldest Peruvian medical journal published nowadays. In order to accomplish this goal we reviewed the past volumes that are part of the invaluable collection of the San Fernando Faculty of Medicine Museum of Health Sciences. Its pages are witness of the development experienced during the past 80 years of biomedical science, as well as the development of Public Health, Medical Education, Medical Ethics, and History of Medicine, therefore the Peruvian Medical School. Since its re-launching in 1995, this journal reflects the professional quality of a number of generations of Peruvian physicians and is one of the most constant periodical journal and aims to take a place that used to have in the past, thus the best Peruvian journal.

The Peruvian University, referring to the University and not genuine centers where higher education recently formed with mercantilist or proselytizing purposes is taught, remains a space for reflection, an institution located within a given historical reality that not only serves base, but is a temporal and spatial situation that must be studied, analysis, evaluation and judgment.

Currently, the public asks for higher guarantees regarding the capacity and entrustness of physicians involved with their care. This latter engage us with the accomplishment of some minimal outlines in order to assure an adequate practice of Medicine. We emphasize here the necessity of a certification that must preserve the capacity of the physician who works in our country. Moreover, we propose the conformation of a National System of Certification for the faculties of Medicine and its goals are pointed out. Besides, we made a review of certification patterns from other countries.

We review the  latest insigths on the understanding of immununosenenscense, or aging of the inmune systeni. This process results from  the interaction between bothl inborn (genetic) and acquired (nutritional status, lifestyle,  and host-parasitic interaction) features. Among genetic features, genetically controled expression of HLA system molecules on inmune cells and possible linkage of some allelies expression to  longer longevity stand out. Other main features are changes of the aged patient inmune system and understand of  of thymus involution as well as changes in the distribution, quality, and quantilty of lympliocyte sub-populations. Nowadays, the University remains to be a place of reflexion, nevertheless inside the historical process, it should play a role as an organization of services because, as an institution, it manages academically information to render individual and general services to the country and community. It is required a managerial and not a mere administrative University which focus its attention toward the total quality as well as the education of executive students and toward the re-estructuration of their roles.
