Objective: To determine if the use of inhaled corticosteroids is a risk factor for community-acquired pneumonia in patients with Chronic Obstructive Pulmonary Disease at the Víctor Lazarte Echegaray Hospital during the period 2017-2020. Methods: The study was analytical, observational, retrospective of cases and unmatched controls in a ratio of 4:1 with a population of 405 subjects from whom 81 cases and 324 controls were selected, who met the selection criteria. Las entidades sinusoidales son raras en reumatología. En la esclerosis sistémica progresiva existe una variante de la forma cutánea limitada denominada esclerosis sistémica sine scleroderma, cuya característica central es la ausencia de afectación cutánea, pero sí afectación visceral. Los anticuerpos antitopoisomerasa o anticentrómero positivos confirman el diagnóstico. Presentamos el caso de una mujer de 63 años con enfermedad pulmonar intersticial, afectación del tránsito intestinal y fenómeno de Raynaud, con títulos elevados de ANA con patrón centromérico y positividad para anticuerpos anti-topoisomerasa. Ante un paciente con fenómeno de Raynaud, compromiso visceral y ANA elevado, se deben solicitar anticuerpos específicos para el diagnóstico de esclerosis sistémica en su variedad sinusal .

La Enfermedad Pulmonar Obstructiva Crónica (EPOC) se caracteriza por una limitación del flujo aéreo espiratorio, tiene un componente sistémico extrapulmonar que puede contribuir a la gravedad. La etiología más importante es el tabaquismo y en su descompensaci´´on intervienen las infecciones. Los broncodilatadores corticoides y antibióticos son los medicamentes mas frecuentes en el tratamiento de la exacerbación de EPOC, sin embargo la VMNI es una opción terapéutica con evidencia sólida en la exacerbación moderada a severa. Una mejoria clínica después de dos horas de prueba de VMNI es predictiva de éxito.

Hereditary PAH (PAH) is defined as a genetic condition of autosomal dominant manner, incomplete penetrance, variable expressivity, and which cases of familial PAH are defined by the presence of two or more family members with PAH with or without an identified germline variant and cases of idiopathic PAH corresponding to isolated cases in the family with an identified germline variant. Introduction: Pulmonary arterial hypertension (PAH) is a low prevalence condition for which there is very little information in Peru. Several studies indicate that it is associated with elevated costs for health systems in the world. Objective: To establish the economic burden of PAH in the Peruvian context. Methods: Information on the use of health resources was collected through consultations with clinical experts and key opinion leaders. Reportamos el caso de una paciente mujer de 20 años con disnea y dolor torácico asociado a esfuerzo moderado, sin enfermedad cardiaca conocida, previamente. Las evaluaciones demostraron dilatación de tronco principal y ramas de arteria pulmonar con regurgitación pulmonar severa, hipertensión pulmonar leve, dilatación leve e hipertrofia moderada de ventrículo derecho; sometida a cirugía por insuficiencia pulmonar severa y cura quirúrgica de aneurisma pulmonar. El hallazgo intraoperatorio fue agenesia del velo septal de la válvula pulmonar y aneurisma de arteria pulmonar y sus ramas. Se realizó un cambio de válvula y plastía del aneurisma.

Clinical monitoring of pulmonary arterial hypertension in our country, in which patients come from different altitudes above sea level, forces us to rule out pulmonary hypertension in relation to chronic exposure to high heights, described in third group of international classification. When reviewing the hemodynamic variations in pulmonary pressure with exercise at altitude with respect to sea level in healthy patients, this is greater in height, this would explain that the patient with pulmonary arterial hypertension is more symptomatic while living at a higher altitude above sea ​​level.

Objective.  Evaluate the change of lactate levels and its prognostic role in the postoperative period of patients undergoing pulmonary thromboendarterectomy. Methods. Retrospective study between 2001 and 2019. Patients older than 18 years and who underwent pulmonary thromboendarterectomy were included. The U Mann Whitney test was performed to evaluate the change between lactate levels, and Cox regression analysis to evaluate the relationship with mortality. Areas under the curve were constructed for lactate levels. Results. Seventy-three patients were operated on during the study period. Median age was 51 years, 55% female. The median lactate on days 1 was 4.65 mml/L and on day 2 it was 1.62 mml/L with a change of 2.87 mml/L.
