Objectives: The aim was to determine the association between tacrolimus levels and early renal graft function, acute rejection and infections in the first month after early kidney transplantation. Methods: Retrospective, observational and analytical study. Review of records of kidney transplant patients over 18 years of age during 2016, who met the inclusion criteria (n = 36). The descriptive and analytical statistics were with STATA 15, significance p <0.05. Results: Age 41.6 ± 15.3, males 52.78%, deceased donor 77.78%, 86.11% had ≤ 3 incompatibilities HLA and PRA 0%. They received induction 97.22%. Tacrolimus dose (TAC C) on the seventh day was 8.7 ± 3.4 ng / ml. The 16.66% were donors of expanded criteria, 22.22% with delayed graft function, acute tubular necrosis (6/36), anticalcineurin toxicity (10/36) and acute cellular rejection IA (1/36). The overall TAC C / D ratio <1....

ABSTRACT Fabry disease is a lysosomal storage disease caused by alpha galactosidase A enzyme deficiency. The pattern of inheritance is X-linked. The clinical presentation has a variety of symptoms and signs; and a classic clinical variant and non-classical clinical variants such as renal or cardiac have been described. The diagnosis is clinical and laboratory and even molecular and in some cases histology of the compromised organ is needed. The treatment in recent years remains symptomatic therapy and with enzyme replacement therapy; although chaperones are already available for the treatment of a group of patients with "amenable" mutations and they are studying gene therapy and nanotechnology. It must have a multidisciplinary approach, given the variety of symptoms and signs in its presentation. Early detection and early initiation of therapy could improve quality of life and limit the ...

Introduction: Kidney failure is one of the most frequent extrapulmonary complications in patients hospitalized with COVID-19, leading to poorer outcomes, and this may have serious consequences for the Peruvian health system. Nonetheless, there are studies comparing patients with acute kidney failure (AKF) and chronic kidney failure (CKF) against healthy subjects. Objective: To determine the clinical characteristics of hospitalized patients with COVID-19 and kidney failure, and to assess the effect of the type of kidney failure and undergo hemodialysis with respect to negative clinical outcomes. Methods: This is a descriptive cohort study that included patients with some kind of kidney failure and COVID-19 who were hospitalizedbetween March and June 2020, and who had a consultation with the nephrology service. Kidney failure was classified as acute, chronic, and stage V chronic undergoing...

Introduction: This article summarizes the clinical practice guide (CPG) for the screening and management of malnourished patients or patients at risk of malnutrition in the Social Security of Peru (EsSalud). Objective: To provide clinical recommendations based on evidence for the screening and management of malnourished patients or patients at risk of malnutrition in EsSalud. Methods: A CPG for the screening, management and monitoring of malnourished patients or patients at risk of malnutrition in EsSalud was developed. To this end, a guideline development group (local GDG) was established, including medical specialists and methodologists. The local GDG formulated 9 clinical questions to be answered by this CPG. Systematic searches of systematic reviews and -when it was considered pertinent- primary studies were conducted in Pubmed and CENTRAL during 2021. The evidence to answer each of ...

Background: This article summarizes the clinical practice guideline (CPG) for the initial pharmacological treatment of lupus nephritis in the Social Security of Peru (EsSalud). Objective: To provide evidence-based clinical recommendations for initial pharmacological treatment of non-refractory adults with class I to V lupus nephritis in EsSalud. Material and Methods: A guideline development group (GDG) was formed, which included specialist physicians and methodologists, who formulated clinical questions. Systematic searches were conducted for systematic reviews and - when deemed relevant - primary studies in PubMed during 2021. Evidence was selected to answer each of the clinical questions posed. The certainty of evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic working meetings, the GEG used the GRADE meth...
