Objectives: To determine the clinical, imaging and laboratory characteristics and one year after diag- nosis survival of patients with cardiac amyloidosis in a national reference hospital. Materials and methods: Case series study. We evaluated the clinical characteristics, complementary examinations and survival of patients with cardiac amyloidosis diagnosed, treated and followed up in the Clinical Cardiology service of the National Cardiovascular Institute – INCOR EsSalud in Lima, Peru. Results: We found eight patients with diagnosis of cardiac amyloidosis. The median age was 64.5 years and 75% were male. The etiology of cases was unspecified cardiac amyloidosis (25%), transthyretin cardiac amyloidosis (37.5%), and light chain cardiac amyloidosis (37.5%). Focal atrial tachycardia is a rare type of supraventricular tachyarrhythmia, generally present in young people, and is a rare cause of tachycardiomyopathy (10%). We present a clinical case of tachycardiomyopathy in a 30-year-old man, without comorbidities, who was diagnosed with incessant focal atrial tachycardia, refractory to medical therapy, and electrical cardioversion. Succesful endocardial ablation was performed, and in outpatient follow-up at 6 months, he showed a recovery of the left ventricular ejection fraction and reduction of the left chambers to normal ranges, with progressive decrease of dyspnea.

General Objective: Determining that there is relationship between typesof child abuse and types of violations in detained teenagers in a YouthCentre of Trujillo. Material and Methods: A descriptive, correlational study and cross sectional were carried out in 117 detained teenagers in the center, who were applied the Childhood Trauma QuestionnaireSurvey-Short Form (CTQ-SF). All of them, only 86 met the inclusion criteria, constituting my test sample. Results: Physical and emotional abuses, physical and emotional neglects are directly related to infractions against property; whi le sexual abuse is inversely related. La taquicardia atrial focal es un tipo de taquiarritmia supraventricular poco prevalente, generalmente presente en jóvenes, y constituye una causa rara de taquicardiomiopatía (10%). Presentamos un caso clínico de taquicardiomiopatía en un varón de 30 años, sin comorbilidades, que fue diagnosticado de taquicardia atrial focal incesante, refractaria a terapia médica y cardioversión eléctrica, por lo que se realizó ablación endocárdica, la misma que fue exitosa y en seguimiento ambulatorio a los 6 meses evidenció una recuperación de la fracción de eyección del ventrículo izquierdo y reducción de cavidades izquierdas a rangos normales, con disminución progresiva de disnea.

Recipients of solid organ transplants are among the risk population due to immunosuppression. There is lack of information about the presentation, clinical course, management, and prognosis of coronavirus disease (COVID-19) in these patients. We present 2 cases of heart transplant recipients who contracted SARS-CoV-2 infection and received treatment in a Cardiology Institute in Lima, Peru.

Desde los primeros reportes de la infección por SARS-CoV-2 en el mes de diciembre de 2019 en Wuhan, China, hemos sido testigos de su propagación mundial y de las graves complicaciones que ha ocasionado en miles de pacientes. En la población de riesgo se encuentran los receptores de trasplantes de órganos sólidos quienes son especialmente vulnerables por su condición de inmuno-suprimidos. Aún no existe suficiente información sobre la presentación, curso clínico, manejo y pronóstico de la enfermedad por coronavirus (COVID-19) en estos pacientes. Presentamos dos casos de pacientes trasplantados cardíacos con infección por SARS-CoV-2 atendidos en un instituto de Cardiología de la ciudad de Lima, Perú.

Objetivo. Determinar las características clínicas, terapéuticas y la mortalidad intrahospitalaria de los pacientes con complicaciones mecánicas posinfarto de miocardio (IM). Materiales y métodos. Estudio observacional, descriptivo y retrospectivo. Se incluyó a pacientes > 18 años con diagnóstico de complicación mecánica pos-IM hospitalizados en el Instituto Nacional Cardiovascular (INCOR) en Lima-Perú, del 1 de enero de 2017 hasta el 31 de diciembre de 2021. Se estudiaron variables clínicas, terapéuticas, complicaciones y mortalidad intrahospitalaria. Resultados. The definition of high-risk chronic coronary syndrome varies depending on the noninvasive test used to trigger ischemia. The triggering occurs through increased myocardial work and oxygen demand, either through exercise or drugs. The initial approach to the chronic coronary syndrome leads us to discuss in which cases to prioritize an optimal initial medical therapy or to perform an initial invasive procedure of myocardial revascularization. In this article we analyze both approaches based on previous studies carried out to date, where the initial invasive management has not been shown to be superior to initial optimal medical therapy in outcomes such as death or major adverse cardiovascular events.

La definición de síndrome coronario crónico de alto riesgo varía según la prueba no invasiva empleada para evidenciar isquemia, lo que se consigue a través de un aumento del trabajo miocárdico y la demanda de oxígeno, generados con el uso de fármacos o el ejercicio. El abordaje inicial nos sumerge en la discusión de contextualizar en qué casos priorizar la terapia médica óptima inicial, frente al manejo invasivo inicial con el objetivo de lograr la revascularización miocárdica. Este aspecto central será desarrollado en esta revisión, tomando en cuenta diversos estudios realizados hasta la fecha, en los que el manejo invasivo inicial no ha demostrado ser superior a la terapia médica óptima inicial en resultados clínicos relevantes como muerte o eventos adversos cardiovasculares mayores.
