Determinar si las alteraciones del perfil de coagulación son factor predictor de severidad en preeclampsia en gestantes del Hospital Regional Docente de Trujillo durante 2022-2024. MATERIAL Y MÉTODOS: se realizó un análisis observacional, analítico y retrospectivo de pruebas diagnósticas en una cohorte anidada. El estudio incluyó 62 gestantes con preeclampsia leve y 62 con severa, entre enero de 2022 y diciembre de 2024. Se analizaron registros completos del perfil de coagulación utilizados rutinariamente en el nosocomio: tiempo de protrombina, tiempo parcial de tromboplastina activado, INR y recuento plaquetario; además de información clínica y obstétrica. Se hallaron puntos de corte mediante curvas ROC y áreas bajo la curva (AUC); la capacidad predictiva de parámetros de forma aislada y combinada se evaluó por la prueba de chi-cuadrado. RESULTADOS: Los puntos de corte ajustado a las curvas ROC fueron para TP (13,05 s), TPTa (36,65 s), INR (0,96) y plaquetas (172 ×10³ cel/mm³). Individualmente, el TP presentó mayor sensibilidad (69,4%), mientras que las plaquetas mayor especificidad (87,1%) y valor predictivo positivo (87,10%). En regresión bivariada, la combinación plaquetas e INR mostró la asociación más fuerte con severidad (OR 7,20; p = 0,012), con alta especificidad (96,8%) y VPP (85,7%). Estos hallazgos confirman la predicción de severidad. CONCLUSIÓN: Las alteraciones del perfil de coagulación ajustados a las curvas ROC, especialmente la combinación de recuento plaquetario e INR predicen severidad en preeclampsia, pero deben incorporarse otros marcadores bioquímicos o inflamatorios para mejorar la precisión diagnóstica de severidad |

Materials and Methods: An observational, analytical, retrospective analysis of diagnostic tests was conducted within a nested cohort. The study enrolled 62 women with mild preeclampsia and 62 with severe preeclampsia between January 2022 and December 2024. Routine hospital records of coagulation profiles—including prothrombin time (PT), activated partial thromboplastin time (aPTT), international normalized ratio (INR), and platelet count—were reviewed alongside clinical and obstetric data. Cutoff points were identified using receiver operating characteristic (ROC) curves and areas under the curve (AUC). The predictive performance of individual and combined parameters was assessed by chi-square testing for sensitivity, specificity, positive predictive value, negative predictive value, and likelihood ratios. Results: ROC-derived cutoff values were 13.05 s for PT, 36.65 s for aPTT, 0.96 for INR, and 172 × 10³ cells/mm³ for platelets. PT exhibited the highest sensitivity (69.4%), whereas platelets showed the highest specificity (87.1%) and positive predictive value (87.1%). In bivariate regression, the platelet–INR combination demonstrated the strongest association with severity (OR 7.20; p = 0.012), with high specificity (96.8%) and positive predictive value (85.7%), confirming their predictive utility.
