We carried out an observational, retrospective and descriptive study in order to identify the clinical and epidemiological characteristics of children with SARS-CoV-2 infection admitted to a Peruvian national referral hospital. We included patients from one month old to fourteen years old hospitalized between March and August 2020. A total of 125 patients with SARS-CoV-2 infection were admitted, 18.4% (n = 23) had critical illness and 16.8% (n = 21) had multisystem inflammatory syndrome (MIS-C). The absence of comorbidities and previous history of epidemiological contact were more frequent in patients with MIS-C. Patients in critical condition and patients with MIS-C had lower lymphocyte and platelet counts, and higher C-reactive protein, ferritin and D-dimer values than patients who did not have said conditions. Background: Septic shock is a common cause of admission to pediatric intensive care units (PICU) and is associated with significant mortality. Our objective was to analyze the association between septic shock diagnosis at PICU admission and mortality during hospitalization. Method: This retrospective cohort study was conducted in the PICU of Hospital Nacional Edgardo Rebagliati Martins (HNERM) in Lima, Peru, from January 2018 to December 2021. The sample size was determined based on annual mortality rates, and randomization was used to create two groups: patients with and without septic shock. Objetive: Report the case of a girl who presented septic shock, hypercalcemia and hemophagocytic syndrome due to paracoccidioidomycosis. Case report: 3-year-old female patient with 2-month illness characterized by fever, lymphadenopathy, weight loss, abdominal distention, and severe anemia. She arrived in poor general condition, developed septic shock and a hemophagocytic syndrome. The hypercalcemia and dermal lesions were striking. A cervical ganglion biopsy and bone marrow aspirate were performed. In the latter, images compatible with Paracoccidioides were found, which were then confirmed with Grocott's stain. She was treated with amphotericin B and trimethoprim-sulfamethoxazole. The evolution was favorable and serum calcium came back to normal ranges. It affects both the adult and pediatric population, however the most published information correspond to the first one, generating a knowledge gap regarding children. Pulmonary involvement appears to be the most frequently manifestation of the disease, although extrapulmonary complications such as pericardial effusion and cerebrovascular diseases have been reported in adults, and according to our review none have been reported in children. La enfermedad por coronavirus 2019 (COVID-19) es una pandemia según declaraciones de la Organización Mundial de la Salud. Afecta tanto a la población adulta como pediátrica; sin embargo, la mayor información publicada corresponde a los primeros, generando una brecha de conocimiento con respecto a los niños. El compromiso pulmonar parece ser la manifestación más frecuente de la enfermedad, aunque se han reportado afecciones extrapulmonares como derrame pericárdico y enfermedades cerebrovasculares en adultos; sin embargo, según nuestra revisión ninguna ha sido reportada en niños. Objetivo: Describir las características clínicas y laboratoriales del derrame pleural paraneumónico y del empiema en niños de 1 a 14 años hospitalizados en el Servicio de Pediatría del HNDAC de Enero 2007 – Diciembre 2013. Materiales y métodos: Estudio descriptivo, retrospectivo, transversal, en pacientes desde 1 mes de edad hasta 14 años con 11 meses y 29 días, que ingresaron al Servicio de Hospitalización de Pediatría del HNDAC con diagnóstico de derrame pleural paraneumónico y/o empiema, desde el 01 de Enero del 2007 hasta el 31 de Diciembre del 2013. Resultados: En los 7 años de estudio hubo 32 casos (74.4% del total) de derrame pleural paraneumónico y 11 (25.6%) de empiema, haciendo una prevalencia de 1.1% y 0.4% respectivamente. Later, the Pediatric Multisystemic Inflammatory Syndrome (SIMS) associated with SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus-2) was described, whose peak incidence was 4 weeks after the maximum peak of infections, so it is postulated that it is a post-infectious entity, and is characterized by affecting several organs and systems. In response to this new need, the Pediatric Intensive Care Unit (PICU) of the Edgardo Rebagliati Martins National Hospital set up a differentiated environment with two beds for the care of children with SARS-CoV-2 infection, to reduce the risk of in-hospital infections.

Hallazgos: Se identificaron 18 niños con edad promedio de 8,76 años, 50% masculino, todos con prueba serológica positiva. Objective: To describe the symptoms, treatment and evolution of SIMP in patients of a reference Pediatric Intensive Care Unit (PICU). Findings: 18 children were identified with a mean age of 8.76 years, 50% male, all with a positive serological test. The most frequent manifestations were: fever (100%), respiratory distress (94%), abdominal pain (89%), elevated C-reactive protein (average 21.8), thrombocytopenia (50%), pathological chest X-ray (89%) and depressed myocardial contractility (61%). 72% required invasive mechanical ventilation (average 3 days) and catecholamines; 89% used corticosteroids plus immunoglobulins.
