OBJECTIVE: To describe our experience in pancreaticoduodenectomies (Whipple resection) performed to resect a number of malignant diseases of the periampullary region, and the mortality as well as the operative time. MATERIALS AND METHODS: A single-institution (Hospital Nacional Almenara I.), retrospective review of the medical records from patients who underwent pancreaticoduodenectomy between 1994 and 1997 inclusive, was performed. RESULTS: We found 9 patients. whose mean age was 69 years, with 56% male and 44% female. The median operative time was 270 min (ranged from 360 to 720 min). Pathologic examination results showed periampullary adenocarcinoma of the head of the pancreas (66,7%, n=6), ampullary adenocarcinoma (11,1%, n=1), duodenal periampullary adenocarcinoma (interstitial tvpe) (11.1%), and distal bile duct primary adenocarcinoma (11,1%). We report the case of a 37-years-old woman, who was treated for a metastatic carcinoma located at the ovary from extragenital source: Krukenberg´s tumor, at the Surgery Department N.º 5, Hospital Nacional Guillermo Almenara Irigoyen - IPSS. The tumor showed signet ring cells in cellular stroma, derivated from ovarian stroma. The primary neoplasm was gastric cancer and has been submitted to total gastrectomy plus omentectomy (R-II Resection) with esophagus-jejuno lateral end anastomosis in Y of Roux with autosuture.After 18 months, she underwents a second intervention: total abdominal hysterectomy and bilateral salpinx oophorectomy, showing ascitis and bilateral enlarged solid ovarian masses. Her survival was 24 months after the first intervention.

The term Krukenberg tumor metastases should be reserved for those containing significant amounts of signet ring cells with intracellular mucin production and diffuse sarcomatoid ovarian stromal reaction. This limitation is important because tumors with these microscopic characteristics also have macroscopic pathologic features and clear clinics. Almost all have metastasized from the stomach, although some come from the breast, bowel or other organs that contain glands mucosas1,2. The aim of this study is to provide data to the national casuistry presenting therapeutic clinical, diagnostic, and prognosis of this tumor.

La distribución del cáncer de colon es variable en su localización anatómica. El objetivo de este tgrabajo es mostrar la correlación hallada entre la distribución anatómica del cáncer de colon con la edad )al momento del diagnóstico) y el sexo de los pacientes. Se realizó un estudio retrospectivo de los casos de pa cientes con cáncer de colon en el Servicio de Cirugía V del Hospital Nacional "Guillermo Almenara Irigoyen" del IPSS, en los últimos cinco años. Se revisó los ñprotocolos operatorios y las historias clínicas, para obtener las variables. edad del paciente al momento del diagnóstico, sexo y localización anatómica de la neoplasia. Se estudiaron 27 pacientes en cinco años (1992-1997), el 52 % (n=14) eran hombres y el 48 % (n=13) mujeres.

From January 1995 to February 1996 in the Department of Surgery at Guillermo Almenara Irigoyen Hospital (General Surgery Service N° 5) we resolved to prepare a protocol of assessment and/or nutritional support in patients who are awaiting for surgery or who recently has undergone surgery. Our porponse is to decrease the morbimortality rate. After several work meetings, we resolved to offer this protocol for its aplication, as well as evaluation and follow up of the patients.
