OBJECTIVE: To describe the clinical characteristics and perinatal maternal complications of HELLP syndrome. DESIGN: Retrospective descriptive. MATERIALS AND METHODS: Cases of HELLP syndrome treated at the National Hospital Guillermo Almenara lrigoyen (HNGAI) in 1999. RESULTS: The incidence of HELLP syndrome was 16.3 per 1,000 live births, 16 (41%) with complete criteria and 23 (58.9 %) with partial HELLP. epigastric and right upper quadrant pain in 46.2% of patients were recorded, partnering with more thrombocytopenia (OR = S. p <0.001), major obstetric complications were DIC 17 (43.6%), acute respiratory failure 14 (35. 9%), placental abruption 9 (23.1%) and eclampsia four (10.3%). complications in partial HELLP were lower. Gestation was completed in 63% of patients between 33 and 37 weeks, 95% by cesarean section. blood products was used in 67%, according to the severity of thrombocyt...

Objective: To determine incidence and results of cesarean-hysterectomy from 1991 through 1998. DESIGN: Obstetrical Service, Guillermo Almenara National Hospital. RESULTS: There were 30851 deliveries, 8746 by cesarean section, incidence 28,3%. Cesarean-hysterectomies represented 0,3% of all cesarean sections or an incidence of 0,94 per 1000 deliveries. Average age was 34,6 years old, 551,7% older women, 20,7 great multiparae, 20,7% nulliparae and 3,6 average parity. Preterm deliveries predominated and previous cesarean section represented, 34,5%. Indications for cesarean section were abruptio placentae, bleeding placenta previa, severe preeclampsia, acute fetal distress, deflection, 2 or 3 previous cesarean sections, prolonged pregnancy, myomatosis, and in situ cervical carcinoma. Indications for hysterectomy were placenta accreta uterine atony, myomatosis, Couvelaire uterus, chorioamnion...

OBJECTIVES: To make a critical evaluation of the performance of Ultrasonographic Biophysical Profile (UBP) as a method for antepartum fetal evaluation; to define their main outcome measures as well as their interpretation, and propose a fetal well-being assessment algorithm which considers the score obtained. MATERIAL AND METHODS: Observational, prospective blind study. Since october, 1997 to september, 1998, 120 women with a gestational age ³28 weeks who required fetal well-being assessment to decide pregnancy interruption, at the High-Risk-Obstetrics Department, Hospital Guillermo Almenara, Lima. We evaluate the difficulties and benefits obtained with the ultrasonographic assessment of baseline fetal heart rate and heart reactivity; and the value of ultrasonographic qualitative assessment of amniotic fluid volume, placentary senescence and early placentary aging; and the value of esta...

The authors present a collaborative study that prospectively has engaged 14 maternity hospitals in the public sector, 10 from the Ministry of Health (MOH) and 4 of the Peruvian Institute of Social Security (IPSS); Likewise 9 corresponded to Lima and 5 establishments were in other regions. Between March and July 1993 he managed to gather a total of 4205 cases of cesarean births attended between 21797 during the same period, representing a rate of 19.3%. There was no major difference in the rate between hospitals in Lima and provinces, but if a higher frequency was found in IPSS when compared with the MOH. The six most important causes of cesarean in Peru are: previous cesareada, acute fetal distress, breech birth, narrow pelvis, fetopelvic disproportion and toxemia, which remain more or less the same distribution between the MOH and IPSS. Some other epidemiological characteristics differ ...
