Sumario:Open appendectomy has been the gold standard in the treatment of acute appendicitis, but currently the laparoscopic technique is proving superior. Surgical interventions in obese patients are complex because of the technical difficulty in visualizing structures and identifying anatomical landmarks. However, local and/or regional studies are required to recognize the differences between the two procedures. OBJECTIVE: To compare the efficacy of laparoscopic versus open appendectomy in patients with BMI ≥ 30 kg/m² at Hospital Belen de Trujillo (HBT). MATERIALS AND METHODS: Ninety-eight obese patients undergoing appendectomy, 49 treated with laparoscopic appendectomy and 49 with open appendectomy, were studied at HBT during the period January 2017 - December 2019. RESULTS: The mean age in the group of patients undergoing laparoscopic appendectomy (LA) was 30.31 ± 9.85 years; while in open appendectomy (OA) it was 34.43 ± 11.49 years. Of the LA patients group, 18 (18%) were female and 31 (32%) male; and of the OA patients group, 19 (19%) were female and 30 (31%) male. The mean operative time of patients operated for AA was 66.84 ± 20.02 minutes, while for LA it was 58.20 ± 16.39; a significant difference was found between both groups (p<0.05). The mean hospital stay of the AA patients group was 4.35 ± 2.72 days and the AL patients group was 3 ± 1.02; a significant difference (p<0.05) was found between the mean hospital stay of both groups. Of The total number of patients undergoing AA or LA, 20.4% suffered postoperative complications. The percentage of patients with complications undergoing AA was 30.6% while the LA group was 10.2%; a significant difference (p<0.05) was found between the proportion of complications in both groups. CONCLUSIONS: Laparoscopic appendectomy in obese patients is more effective than open appendectomy with respect to operative time, hospital stay and postoperative complications.
