Multiple Sclerosis is a chronic, demyelinating, autoimmune, neuroinflammatory disease. Known as the most common cause of non-traumatic neurological disability in young adults. Ten per cent of patients with Multiple Sclerosis are diagnosed with the Primary Progressive form (PPMS) which, until the emergence of the anti-CD20 monoclonalantibody Ocrelizumab, had no specific therapy. The first case treated with Ocrelizumab in the Peruvian publichealthcare system is reported. The patient presented an acceptable tolerability and an adequate clinical response, as measured by the EDSS scale. Of note, under Peruvian legislation, Multiple Sclerosis is considered a rare diseaseand, therefore, requires an ad hoc evaluation for the authorization of public funding for specific therapies.

Countries do plenty of effort in order to implement the objectives for sustainable development, aiming to achieve universal health coverage. However, access or chances for having high quality Clinical Pathology services are both necessary for supporting medical care systems if such objectives are to be achieved. This access is much more difficult to achieve in low- and medium- income countries. This paper presents an analysis of both the global and local situation in this respect. Four key barriers for delivering high quality and optimal services were identified: insufficient human resources or limited capability of the working force, inadequate education and training, inadequate infrastructure and equipment, and insufficient quality, regulations, and accreditation strategies.

The impact of the global COVID-19 pandemic on health systems has created a chaotic scenario, severely hindering the ability to meet health needs and negatively affecting the provision of health services to the population. This prolonged health crisis has exposed the collapse of health systems and their lack of institutional capacity to effectively address health emergencies. This situation is largely attributable to the segmentation and fragmentation rooted in the regional structure of these systems. Meanwhile, limited progress has been made in evaluating their performance. One of the primary objectives of health systems is universal health coverage. Nevertheless, achieving this goal requires the implementation of strategies aimed at strengthening these systems through the evaluation of their performance and the promotion of international health security. In this context, essential publi...

Cardiovascular risk is poorly managed in women, especially during the menopausal transition when susceptibility to cardiovascular events increases. Clear gender differences exist in epidemiology, symptoms, diagnosis, progression, and management of cardiovascular risk. Key risk factors to be controlled in the peri-menopausal woman are hypertension, dyslipidemia, obesity, and other components of the metabolic syndrome, with the avoidance and careful control of diabetes. Hypertension is a particularly powerful risk factor and lowering of blood pressure is pivotal. There are some scores for risk stratification and Reynolds’ is of relevant significance. Team work is of extreme importance in the optimal risk cardiovascular stratification in women, moreover in the perimenopausal period.

Research stands as a fundamental pillar that recognizes the university as an entity of institutional and social relevance. This statement is particularly true in the field of health sciences, where educational institutions are expected not only to generate knowledge for the benefit of society but also to shape individuals capable of immersing themselves in research and strengthening this vital premise. In this context, intricate concepts that outline the nature and purpose of research in the academia emerge. Scientific research, on the one hand, arises as the driving force behind the advancement of knowledge, emphasizing rigorous and systematic methods to unravel the mysteries of reality. On the other hand, research training stands as a pivotal educational vehicle for nurturing analytical and curious minds, providing future professionals with the necessary tools to inquire and contribute...

The Primary Health Care strategy introduced concepts which sought to revolutionize the way how to achieve Health for All at the global level. The Declaration of Alma Ata was a prelude to initiatives and global plans that sought compromise to States and society in achieving access to health equity. The Summit of Action for Children and the Meeting of the Millennium, which agreed health targets to achieve by year 2015 were inspired by the concepts included in Primary Health Care. While it is true that the purposes lying below the postulates in Alma Ata were not reached, there was remarkable progress in aspects related to the Mother and Child Health. The Selective Primary Healthcare inspired the so-called “Revolution for the Child Survival”, which identified the main causes of the 15 million deaths in children recorded at the global level at the beginning of the Decade of the eighties o...

Costa Rica has presented progress in oral health indicators in the last 10 years due to its National Health System structure. Health care services are divided into public, private and mixed services. The Costa Rican Social Security Fund is the entity that dominates the public sector and is the main one, divided into three tiered levels of care. This article seeks to expose the structure of the institutions that make up the surveillance and care of the most prevalent oral diseases in the Costa Rican population. In the same way, it presents the progress of these conditions through the last years. It is concluded that, despite the progress made, inter-institutional strengthening is necessary to face the challenges in the current oral health situation.

This research analyzes the importance of the construction of Dos de Mayo Hospital in the mid-nineteenth century in the progress and improvement of health in Lima. The need to build it arose as a result of the increase in the population in the city, the growth in the number of patients, the arrival of yellow fever at the beginning of 1868 and the lack of comfort in the hospitals in operation. Dos de Mayo represented the strengthening of the professional and scientific sense of institutions of this type. Through the review of hemerographic sources of the time, the concern to reform the hospital service and improve patient care is observed; in other words, to guarantee one of the fundamental rights of every human being, the right to life.

In the United States traditionally many children have gone unserved. This has been partly due to barriers in accessing services and the result of a fragmented and disorganized system of care. However, in the last two decades substantial progress has been made in adressing these problems. This article reviews the need for children's mental health services in the United States, and describes recent attempts to improve coordination and access to the children's mental health service system. The introduction of systems of care, the expansion of school-based services, and innovations in the provision of community services are discussed.

The concept of entrustable professional activities emerged as an attempt to overcome some of the criticisms to the competency-based medical education approach; it has had a broad impact in practice and health professions education research. It has been disseminated internationally with its English acronym: EPA. This approach proposes to orient assessment and teaching to specific activities in the profession, which allows the integration of several competencies, and to determine which responsibilities can be entrusted to the trainee, in a gradual and explicit manner. The model assumes the definition of levels of supervision that allow progressive autonomy for each EPA, in students or residents, once they demonstrate the required competencies. Practice, supervision and feedback in real clinical scenarios are key to the development of autonomy in EPA performance. The dissemination of the EP...

The policies on children´s health in Peru began as the Peruvian State assumed it function as the main actor of the health system.
