Material y método: La investigación fue observacional, con criterio analítico, y diseño de pruebas diagnósticas; con una muestra de 150 pacientes con diagnóstico de apendicitis aguda según criterio post operatorio; el análisis inferencial se hizo con la prueba de chi cuadrado, además del cálculo de sensibilidad, especificidad, valor predictivo positivo, valor predictivo negativo y área bajo la curva para la prueba en estudio. Resultados: De los 150 pacientes estudiados, con apendicitis complicada se halló 96 (64%) y no complicada 54 (36%). El mejor punto de corte determinado para el diagnóstico fue de 7,86, además se encontró una sensibilidad 88,5%, especificidad 77,8%, valor predictivo positivo 87,6% y negativo de 79,2%. El área bajo la curva fue de 0.892. La procedencia, hiperglucemia y leucocitosis tuvieron asociación significativa con la presencia de apendicitis complicada. Conclusión: El índice neutrófilo-linfocito (INL) demostró alta eficacia diagnóstica para detectar apendicitis aguda complicada, con un punto de corte óptimo (7,86) que respalda su utilidad clínica en el diagnóstico diferencial

Method: The research was observational, with analytical criteria, and design of diagnostic tests; with a sample of 150 patients with diagnosis of acute appendicitis according to postoperative criteria; the inferential analysis was made with the chi- square test, in addition to the calculation of sensitivity, specificity, positive predictive value, negative predictive value and area under the curve for the test under study. Results: Of the 150 patients studied, 96 (64%) had complicated appendicitis and 54 (36%) had uncomplicated appendicitis. The best cut-off point determined for the diagnosis was 7.86, in addition, sensitivity 88.5%, specificity 77.8%, positive predictive value 87.6% and negative 79.2% were found. The area under the curve was 0.892. The origin, hyperglycemia and leukocytosis were significantly associated with the presence of complicated appendicitis.
