Demostrar que el índice FIB-4 alto es un factor asociado a hemorragia digestiva alta variceal en pacientes con cirrosis hepática descompensada con várices esofágicas del Hospital Regional Docente de Trujillo. MATERIAL Y METODO: Investigación de tipo observacional, analítica, retrospectiva, estudio transversal. Contando con una población de 219 pacientes con el diagnóstico de cirrosis hepática descompensada con varices esofágicas quienes ingresaron a la unidad de gastroenterología del Hospital Regional Docente de Trujillo cumpliendo de manera adecuada los criterios de selección, calculando posteriormente su grado de asociación mediante análisis multivariado obteniendo significancia estadística con un valor de p <0.05. RESULTADOS: Entre los hallazgos, se encontró que, un índice FIB-4 ≥ 3.91 se asoció significativamente con HDAV, teniendo un ORa de 17.50 (IC95% 17.49 – 38.36; p=<0.001), subrayando su relevancia como predictor clínico. Otra de las variables asociadas en esta regresión logística fue el grado de várices esofágicas, siendo los pacientes que presentan un grado de várices III o IV se asociaron significativamente con HDAV (ORa 4.12 IC95% 1.90-8.92) CONCLUSIONES: El índice FIB-4 con valor ≥ 3.91 se identificó como un factor asociado significativo a la hemorragia digestiva alta variceal en pacientes con cirrosis hepática |

MATERIAL AND METHOD: This is an observational, analytical, retrospective, cross-sectional study. The study population consisted of 219 patients diagnosed with decompensated liver cirrhosis with esophageal varices who were admitted to the gastroenterology unit of the Regional Teaching Hospital of Trujillo, meeting the selection criteria adequately. The degree of association was subsequently calculated using multivariate analysis, obtaining statistical significance with a p- value <0.05. RESULTS: Among the findings, it was found that a FIB-4 index ≥ 3.91 was significantly associated with HDAV, with an adjusted OR (ORa) of 17.50 (95% CI: 17.49 – 38.36; p < 0.001), highlighting its relevance as a clinical predictor. Another variable associated in this logistic regression was the degree of esophageal varices, where patients with grade III or IV varices were significantly associated with HDAV (ORa 4.12, 95% CI: 1.90-8.92).
