Se extrajeron datos sobre mortalidad a 30 días, lesiones de la vía biliar, tiempo operatorio, conversión a cirugía abierta, reoperaciones, readmisión hospitalaria y estancia postoperatoria. Se utilizaron modelos de efectos fijos o aleatorios según la heterogeneidad estadística (I²). Resultados: Se incluyeron 12 estudios con un total de 4,010,200 pacientes. No se observaron diferencias significativas en la mortalidad a 30 días (RR: 0.98; IC95%: 0.85–1.14) ni en la tasa de lesiones de la vía biliar (RR: 1.49; IC95%: 0.28–7.86). La técnica robótica presentó menor tasa de conversión a cirugía abierta (RR: 0.37; IC95%: 0.28–0.49) y menores tasas de readmisión (RR: 0.87; IC95%: 0.77–0.99), a costa de un mayor tiempo operatorio (DMP: 4.66 minutos; IC95%: 1.14–8.17). La estancia hospitalaria fue discretamente menor en el grupo robótico (DMP: -0.39 días; IC95%: -0.77 a -0.02). Conclusiones: Ambas técnicas son seguras y eficaces. La colecistectomía robótica ofrece ventajas en términos de menor conversión y reingresos, pero requiere mayor tiempo quirúrgico y mayor infraestructura. Materials and Methods: A systematic review and meta-analysis was conducted in accordance with PRISMA guidelines. Searches were performed in PubMed, Embase, and Web of Science up to April 2025. Comparative studies reporting perioperative outcomes of both techniques were included. Extracted outcomes included 30-day mortality, bile duct injury, operative time, conversion to open surgery, reoperations, hospital readmissions, and postoperative length of stay. Fixed or random-effects models were applied depending on statistical heterogeneity (I²). Results: A total of 12 studies comprising 4,010,200 patients were included. No significant differences were found in 30-day mortality (RR: 0.98; 95% CI: 0.85–1.14) or bile duct injury rates (RR: 1.49; 95% CI: 0.28–7.86). Robotic cholecystectomy was associated with a lower rate of conversion to open surgery (RR: 0.37; 95% CI: 0.28–0.49) and lower hospital readmission rates (RR: 0.87; 95% CI: 0.77–0.99), at the expense of a longer operative time (MD: 4.66 minutes; 95% CI: 1.14–8.17). Postoperative length of stay was slightly shorter in the robotic group (MD: -0.39 days; 95% CI: -0.77 to -0.02). Conclusions: Both techniques are safe and effective. Robotic-assisted cholecystectomy offers some advantages regarding conversion and readmission rates, although it requires longer operative time and greater technological resources.
