Materiales y métodos: Estudio de tipo casos y controles, realizado en 174 pacientes con diagnóstico de VIH mayores de 18 años que iniciaron TARGA. Se compararon 34 casos con falla virológica y 136 controles sin falla. Se evaluaron variables demográficas, tratamiento, y comorbilidades. Resultados: La proporción de pacientes con falla virológica fue del 20.1 %. La adherencia irregular al tratamiento se asoció significativamente con la falla virológica (OR = 6.0; IC95 %: 3.5–10.3; p<0.001). También se hallaron asociaciones significativas en el análisis bivariado con el tipo de régimen terapéutico (p=0.039), el recuento basal de CD4 (p = 0.007) y la presencia de comorbilidades (p=0.002), aunque estas no se mantuvieron significativas tras el ajuste multivariado. La edad y sexo no mostraron asociación independiente con la falla virológica.

Objective: To identify the factors associated with virological failure in HIV patients treated at José Cayetano Heredia Regional Hospital during 2023–2024. Materials and Methods: A case-control study was conducted among 174 HIV diagnosed patients aged over 18 years who initiated ART. Thirty-four cases with virological failure and 136 controls without failure were compared. Demographic, treatment-related, and comorbidity variables were evaluated. Results: The proportion of patients with virological failure was 20.1%. Irregular adherence to treatment was significantly associated with virological failure (OR = 6.0; 95% CI: 3.5–10.3; p < 0.001). Significant associations were also found in the bivariate analysis with the type of therapeutic regimen (p = 0.039), baseline CD4 count (p = 0.007), and the presence of comorbidities (p = 0.002), although these did not remain significant after multivariate adjustment. Age and sex showed no independent association with virological failure.
