Materiales y métodos: se realizó de un estudio observacional, de tipo pruebas diagnósticas y retrospectivo, que incluyó a 97 pacientes atendidos por escroto agudo entre los años 2005 y 2023 en el Hospital Regional Docente de Trujillo y Hospital Belén de Trujillo. La puntuación TWIST, constituida por cinco parámetros semiológicos, fue calculada en base a los datos recolectados de los registros clínicos y contrastada con el diagnóstico final de cada paciente. Resultados: la puntuación TWIST mostró una sensibilidad del 78,9% (IC 95%: 54,4-93,9%) y una especificidad del 93,6% (IC 95%: 85,7-97,9%). El VPP fue del 80,4% (IC 95%: 63,0-90,8%) y el VPN del 93,0% (IC 95%: 84,8-97,0%). Se obtuvo un área bajo la curva ROC de 0,899 (IC 95%: 0,807-0,991), indicando un excelente poder discriminativo para diferenciar entre pacientes con y sin torsión testicular. Conclusiones: se logró determinar retrospectivamente que la puntuación TWIST es una herramienta útil en el diagnóstico de torsión testicular en pacientes que presentan síndrome escrotal agudo. Sin embargo, se necesita que la ecografía Doppler testicular continúe complementando la evaluación de estos pacientes

The aim of this study was to determine the usefulness of the TWIST score for the diagnosis of testicular torsion in patients with acute scrotum. Materials and methods: This was an observational, retrospective study of diagnostic testing type, which included 97 patients treated for acute scrotum between 2005 and 2023 at the Regional Teaching Hospital and the Belén Hospital in Trujillo, Perú. The TWIST score, consisting of five semiological parameters, was calculated based on the data collected from clinical records and compared with the final diagnosis of each patient. Results: The TWIST score showed a sensitivity of 78.9% (95% CI: 54.4-93.9%) and a specificity of 93.6% (95% CI: 85.7-97.9%). The PPV was 80.4% (95% CI: 63.0-90.8%) and the NPV was 93.0% (95% CI: 84.8-97.0%). An area under the ROC curve of 0.899 (95% CI: 0.807-0.991) was obtained, indicating an excellent discriminative power to differentiate between patients with and without testicular torsion. Conclusions: The TWSIT score was retrospectively found to be a useful tool in the diagnosis of testicular torsion in patients with acute scrotal syndrome.
