Se analizaron variables demográficas como sexo y edad, variables clínicas como edad, Child-Pugh, sepsis, hiponatremia y deshidratación. Se emplearon pruebas chi cuadrado o Fisher, U de Mann- Whitney, y regresión logística binaria para estimar ORa con IC 95%. Resultados: La adherencia al tratamiento con lactulosa se asoció significativamente con menor recurrencia de encefalopatía hepática (ORa = 0,27; IC95%: 0,09–0,81; p = 0,020). Asimismo, factores clínicos como la presencia de hiponatremia mostró una fuerte asociación con mayor recurrencia (ORa = 6,63; p < 0,001), al igual que la clasificación Child- Pugh B/C (ORa = 4,76; IC95%: 1,75–12,57; p = 0,006). Entre los pacientes con recurrencia, las principales dificultades de adherencia fueron: dosificación incorrecta (68,6%), falta de entendimiento del tratamiento (58,6%) y abandono por sentirse mejor (54,3%). Conclusión: La adherencia a lactulosa constituye un factor protector sólido frente a la recurrencia de encefalopatía hepática. Estos resultados conllevan a fortalecer estrategias educativas y de seguimiento para mejorar la adherencia

Materials and methods: A retrospective, observational case-control study was conducted. One hundred and forty patients with a history of hepatic encephalopathy were included, divided into 70 with recurrence and 70 without recurrence. Therapeutic adherence was assessed using the Morisky test. Clinical variables such as age, Child-Pugh score, sepsis, hyponatremia, and dehydration were analyzed. Chi-square or Fisher's exact tests, Mann-Whitney U tests, and binary logistic regression were used to estimate adjusted odds ratios (OR) with 95% confidence intervals (CI). Results: Adherence to lactulose treatment was significantly associated with a lower recurrence of hepatic encephalopathy (aOR = 0.27; 95% CI: 0.09–0.81; p = 0.020). The presence of hyponatremia showed a strong association with a higher recurrence rate (aOR = 6.63; 95% CI: 4.54–19.23; p < 0.001), as did a Child-Pugh B/C classification (aOR = 4.76; 95% CI: 1.75–12.57; p = 0.006). Conclusions: Adherence to lactulose is a strong protective factor against the recurrence of hepatic encephalopathy.
