Se realizó un muestreo aleatorio simple excluyéndose a menores de 18 años, pacientes con ASA IV o V, cirrosis hepática, cáncer (Ca) vesicular, Ca de vías biliares y Ca de páncreas, así como también aquellos que presentaron datos incompletos en historias clínicas. La variable dependiente es Colecistectomía Laparoscópica Difícil. Se utilizó estadística descriptiva e inferencial, empleando la regresión de Poisson para calcular razones de prevalencia cruda en el análisis bivariado y ajustada en el análisis multivariado, ambos con intervalos de confianza al 95% y un nivel de significancia p<0.05. Resultados: De los 150 pacientes, el promedio del tiempo de enfermedad fué de 10 días (RI: 2 – 90), y en el 98.67% (n=148) se realizó colecistectomía total. Con relación a colecistectomía laparoscópica difícil (CLD), el tiempo de cirugía tuvo una mediana de 60 minutos (RI: 30-150) y la conversión se dió en el 6.67% (n=5) de los casos. En el análisis bivariado se reportó asociación para el tiempo de cirugía (RPc: 1.02; IC: 1.01-1.03) y de la misma forma en el multivariado donde también se encontró significancia estadística al tiempo de cirugía (RPa: 1.03; IC: 1.01-1.04). Conclusiones: El tiempo de cirugía es un factor intraoperatorio asociado a colecistectomía laparoscópica difícil.

Materials and Methods: An observational, analytical-cross-sectional study was performed in 150 cholecystectomized patients. A simple random sampling was carried out, excluding those under 18 years of age, patients with ASA IV or V, liver cirrhosis, gallbladder cancer (Ca), bile duct cancer and pancreatic cancer, as well as those who presented incomplete data in medical records. The dependent variable was Difficult Laparoscopic Cholecystectomy. Descriptive and inferential statistics were used, using Poisson regression to calculate crude and adjusted prevalence ratios in the bivariate and multivariate analysis respectively, with 95% confidence intervals and a significance level of p<0.05. Results: From 150 patients, the mean of illness time was 10 days (IRQ: 2-90) and 98.67% (n=148) were tributaries of total cholecystectomy. Regarding difficult cholecystectomy (DLC), the surgery time had a median of 60 minutes (IQR: 30-150) and conversion occurred in 6.67% (n=5) of cases. In the bivariate analysis, an association is reported for the time of surgery (PRc: 1.02; CI: 1.01- 1.03) and in the same way for the multivariate where statistical significance is also found for the time of surgery (PRa: 1.03; CI: 1.01-1.04).
