The medical prescription is a legal document that must be written in legible letters and a good spelling, doctors prescribe the medication that will be dispensed by pharmacists, or that will be administered by the nursing professional, which has a function important when administering medications (where this activity is carried out day by day). The purpose of this research work is to determine the percentage of prescription errors in a medical service of a Level III-1 hospital in the province of Trujillo, La Libertad department, during the period from September 2018 to February 2019. The absolute frequency, the percentage relative frequency were determined and trend lines were plotted to see the evolution of prescription errors. The results found in the prescriptions were: 1% (59) presented an error in the act of hospitalization and 2% presented an error regarding last names and names; of the medicine, 9% (536) presented omission with respect to the International Common Denomination, 14% (842) in quantity, 7% (386) in administration, 7% (382) in frequency, 10% ( 581) in concentration, 25% (1470) in pharmaceutical form; of the recipe, 11% (646) in use of unapproved abbreviations, 3% (152) in use of inappropriate format
