Los factores de no adherencia incluyeron desabastecimiento de medicamentos, olvido, desconocimiento de la enfermedad, regímenes complejos y síntomas depresivos. La monoterapia mostro mejor adherencia. Una mayor adherencia se asocio con mejor control glucémico y tensional. Conclusión: la adherencia es subóptima y multifactorial. Se necesitan intervenciones integrales que aborden el acceso de medicamentos, la simplificación de tratamientos y la educación del paciente.

Materials and methods: a systematic review (PRISMA-2020) was conducted in PubMed, Scopus, SciELO, and other databases (2020-2025). Of 530 records identified, 14 studies were included. Results: adherence varied widely (14%-90.1%). Factors contributing to non-adherence included medication shortages, forgetfulness, lack of knowledge about the disease, complex regimens, and depressive symptoms. Monotherapy showed better adherence. Greater adherence was associated with better glycemic and blood pressure control. Conclusion: adherence is suboptimal and multifactorial.
