The aim of this study was to describe the endodontic management of three-rooted first and second maxillary premolars with three independent root canals. The approach was performed under magnification, the instrumentation was hybrid and the obturation was performed with a calcium silicate-based sealant. Several cases of this anatomical variation have been reported, therefore a literature review was also carried out to evaluate the presentation and clinical management. Clinicians should be aware of anatomical aberrations that could become evident during the preoperative and intraoperative treatment. Magnification and cone beam computed tomography will play an important role in the long-term prognosis of these cases.Las variaciones anatómicas del sistema de canales radiculares son poco frecuentes, sin embargo, los premolares son los que presentan más variabilidad en comparación con otros dientes. El objetivo del presente artículo fue describir el manejo endodóntico de primeros premolares maxilares con tres raíces y tres canales radiculares independientes. El abordaje se realizó bajo magnificación, la instrumentación fue híbrida y la obturación se realizó con sellador a base de silicato de calcio. Se han reportado varios casos sobre esta variación anatómica, por lo cual también se realizó una revisión de la literatura para evaluar la presentación y el manejo clínico. Los clínicos deben ser conscientes sobre las aberraciones anatómicas, que pueden manifestarse en el preoperatorio e intraoperatorio.
