description Objective: To determine the presence of insulin resistance (IR) in a group of overweight and obese adolescents. Overweight and obesity were determined by Must et al. criteria: overweight between 85 and 95 percentile and obesity overt the 95 percentile. Glycemia and basal insulin were determined in serum obtained by vein puncture. IR was determined by HOMA’s index considering Keskin diagnosis value of 3,1. Results: There was no biochemical statistical difference between the overweight and obesity groups; 14% showed IR, 16,2% in the obese children and 12,3% in those overweight. Average insulin and HOMA in obese children with IR were 315 uUl/mL and 7, respectively. The insulin and HOMA difference between those with and without IR was statistically significant. (p<0,0001). Waist circumference below the median was found in 16% with IR and 12% in the other group. Conclusions: Most of the overweight and obese adolescents in the group studied did not have insulin resistance and only 14% presented IR -16,2% in the obese and 12,3% in those overweight. The existence of IR in adolescents implies higher cardiovascular risk in those overweight and obese and requires preventive measures to face this problem.

Materiales y Métodos: Se estudió 100 niños, comprendidos entre los 10 a 18 años, 77% del género femenino. El 67% presentó sobrepeso y el restante obesidad. Se diagnostícó sobrepeso y obesidad usando la clasificación de Must y col., con los criterios de sobrepeso, entre 85 y 95 percentil, y obesidad, mayor al 95 percentil. Se dosó la glicemia e insulina basal en sangre obtenida por venopuntura. La determinación de la RI se hizo mediante el índice HOMA, considerando como valor diagnóstico el dado por Keskin, de 3,1. Resultados: No existió diferencia estadísticamente significativa de las variables bioquímicas, entre los grupos de sobrepeso y obeso. El 14% de la población estudiada presentó RI. El 16,2% de los obesos y 12,3% de los con sobrepeso tuvieron RI. En los obesos con RI, el promedio de insulina fue 31,5 uUI/mL y de HOMA, 7. La diferencia del promedio de insulina y HOMA entre aquellos con RI y aquellos sin RI es estadísticamente significativa (p<0,0001). Con relación a la circunferencia de la cintura, en el grupo que estuvo por debajo de la mediana, hubo 16% con RI, mientras que en el otro grupo hubo 12%. Conclusiones: La mayoría de adolescentes con sobrepeso y obesidad no tuvo resistencia a la insulina en el grupo estudiado y solo 14% la presentó -16,2% de los obesos y 12,3% de aquellos con sobrepeso.
