Introduction: Melanoma is a highly immunogenic neoplasm whose treatment has evolved with the development of innovative immunotherapeutic strategies. Objective: To analyze advances in immunotherapy and translational medicine for the treatment of melanoma, highlighting new therapies and their impact on clinical practice. Methodology: A narrative review was carried out in scientific databases, including Medline, PubMed, EMBASE, Cochrane Library, OVID and ClinicalKey. Articles were taken in English and Spanish, without time restriction. Relevant studies on immunotherapy, cell therapy, oncolytic viruses and personalized vaccines were selected. Results: The combination of immune checkpoint inhibitors with emerging therapies has improved tumor response and survival. However, tumor heterogeneity and immunosuppression of the microenvironment remain challenges. Conclusion: Translational medicine is key to optimizing melanoma treatment. Interdisciplinary research and biomarker development will allow more effective and personalized therapies, improving clinical outcomes in this pathology. Metodología: Se realizó una revisión narrativa en bases de datos científicas, incluyendo Medline, PubMed, EMBASE, Cochrane Library, OVID y ClinicalKey. Se tomaron artículos en inglés y español, sin restricción temporal. Se seleccionaron estudios relevantes sobre inmunoterapia, terapia celular, virus oncolíticos y vacunas personalizadas. Resultados: La combinación de inhibidores de puntos de control inmunitario con terapias emergentes ha mejorado la respuesta tumoral y la supervivencia. No obstante, la heterogeneidad tumoral y la inmunosupresión del microambiente siguen representando desafíos. Conclusión: La medicina traslacional es clave para optimizar el tratamiento del melanoma. La investigación interdisciplinaria y el desarrollo de biomarcadores permitirán terapias más efectivas y personalizadas, mejorando los desenlaces clínicos en esta patología. Foram considerados artigos em inglês e espanhol, sem restrição temporal. Selecionaram-se estudos relevantes sobre imunoterapia, terapia celular, vírus oncolíticos e vacinas personalizadas. Resultados: A combinação de inibidores de pontos de verificação imunológica com terapias emergentes tem melhorado a resposta tumoral e a sobrevida. No entanto, a heterogeneidade tumoral e a imunossupressão do microambiente continuam a representar desafios. Conclusão: A medicina translacional é fundamental para otimizar o tratamento do melanoma.
