Conclusions: human action and decision is always conditioned, although it has a margin to act as it should and not as it should. In health services, this margin implies that the doctor abandons his protocol commitment and acts according to his free will without delimiting possible responsibilities in the consequences that these could have on patients. The acting of the doctor regulated by the protocols of medicine and deontology emerges in the patient an absolute confidence, therefore, there is no reason for intervention of the ethical and deontological surveillance committee, although, if it were the opposite, it should do so. Objetivo: Reflexionar acerca del actuar médico y el libre albedrío. Método: revisión bibliográfica narrativa-reflexiva. Conclusiones: la acción y decisión humana siempre está condicionada, aunque posee un margen para actuar como le conviene y no como se debe. En los servicios de salud, este margen implica que el médico abandona su compromiso protocolar y actúa de acuerdo a su libre albedrío sin delimitar posibles responsabilidades en las consecuencias que estas podrían tener en los pacientes. El actuar del médico regulado por los protocolos de la medicina y deontología emerge en el paciente una absoluta confianza, por consiguiente, no hay motivo de intervención del comité de vigilancia ética y deontológica, aunque, si fuera lo contrario debería hacerlo. Palabras clave: Deontología, Humanismo, Libertad (Fuente: DeCS, BIREME).  Nos hospitais públicos existe uma alta demanda por serviços de saúde, e os profissionais de diversas especialidades desempenham um papel vital para atender a essa demanda. Objetivo: Refletir sobre a prática médica e o livre-arbítrio. Método: Foi realizada uma revisão bibliográfica de caráter narrativo-reflexivo. Conclusões: A ação e a tomada de decisão humana são sempre condicionadas, embora possuam uma margem que permite ao indivíduo agir conforme sua conveniência e não necessariamente como deveria. Nos serviços de saúde, essa margem implica que o médico pode abandonar seu compromisso com os protocolos e agir de acordo com seu livre-arbítrio, sem delimitar claramente as responsabilidades pelas consequências que tais ações podem ter nos pacientes. Quando a prática médica é regulada pelos protocolos da medicina e pela deontologia profissional, emerge no paciente uma confiança absoluta; portanto, não há necessidade de intervenção do comitê de vigilância ética e deontológica.
