It was observed a delayed eruption of tooth 11. Panoramic and periapical radiographs showed a mixed radiolucent and radiopaque lesion compatible with compound odontoma. A conservative alveolar approach was performed under infiltrative local anesthesia, through a scalloped marginal incision. Then, decollation, conservative osteotomy and excision of three denticles were performed. Likewise, a careful curettage was carried out to prevent injury to vascular structures and dental germ. Subsequently, clinical, and radiographic controls were carried out at 1, 3 and 6 months after surgery; and it was found a favorable evolution. Conclusion: Early diagnosis and conservative surgical approach to compound odontoma were important to prevent retention and non-eruption of permanent teeth in a 9-year-old pediatric patient. The clinical and radiographic control after 6 months follow-up showed a favorable result of the treatment.Objetivo: Revisar información relacionada a las técnicas de distracción utilizadas en odontopediatría para la atención de niños de 5 a 10 años. Reporte de caso: LPaciente de sexo femenino de 9 años y 11 meses sin antecedente de traumatismo dentoalveolar acudió a la Clínica Odontológica de la Universidad Científica del Sur, Lima-Perú. Clínicamente se observó el retardo de erupción de la pieza 11. La radiografía panorámica y periapical mostraron una lesión mixta radiolúcida y radiopaca compatible odontoma compuesto. Se realizó un abordaje alveolar conservador bajo anestesia local infiltrativa, mediante una incisión tipo marginal festoneada. Luego se realizó el decolado, osteotomía conservadora y la exéresis de tres dentículos. Asimismo, se efectuó un curetaje cuidadoso para prevenir lesión de estructuras vasculares y del germen dental. Posteriormente, se realizaron controles clínicos y radiográficos al mes, 3 y 6 meses de la cirugía encontrándose una evolución favorable. Conclusión: El diagnóstico temprano y el abordaje quirúrgico conservador del odontoma compuesto fueron importantes para prevenir la retención y falta de erupción de las piezas permanentes en un paciente pediátrico de 9 años.
