In recent years, numerous cases of congenital malformations and intrauterine growth retardation in neonates of mothers exposed to this drug during pregnancy have been published. We present the case of a fetus with agenesis of both upper limbs in a mother exposed to topiramate during the first trimester, without other associated factors or another possible causal agent detected.El topiramato es un agente farmacológico antiepiléptico de nueva generación, empleado para el tratamiento de crisis parciales y tónico-clónicas primarias en adultos y niños, en monoterapia o en combinación con otros fármacos, y como profilaxis de la migraña en adultos, así como para el alcoholismo, la obesidad, y el trastorno bipolar entre otros. En los últimos años, han sido publicados numerosos casos de malformaciones congénitas y retraso de crecimiento intrauterino en neonatos de madres expuestas a este fármaco durante la gestación. Se presenta el caso de un feto con agenesia de ambos miembros superiores en una madre expuesta a topiramato durante el primer trimestre, sin otros factores asociados ni otro posible agente causal detectado.O topiramato é um agente farmacológico antiepiléptico de nova geração, utilizado no tratamento de crises tônico-clônicas parciais e primárias em adultos e crianças, em monoterapia ou em combinação com outros medicamentos, e como profilaxia de enxaqueca em adultos, bem como para alcoolismo, obesidade e transtorno bipolar, entre outros. Nos últimos anos, foram publicados numerosos casos de malformações congênitas e retardo de crescimento intrauterino em neonatos de mães expostas a esse medicamento durante a gravidez.
