In EID the dissection is performed between the inner (circular) and outer (longitudinal) layers of the muscularis propia. We present the case of a patient successfully treated with EID technique. 54-year-old female patient with a history sessile adenomatous lesion of the rectum of approximately 40 mm resected with conventional endoscopic mucosal resection technique. She presented a recurrence of the lesion on the prior resection scar. EID was performed on the scar of prior endoscopic resection, accessing and exposing the intermuscular plane. En bloc resection of the lesion was achieved with negative histological lateral and deep margins, fulfilling criteria for curative resection. Endoscopic resection techniques offer advantages over radical proctectomy, allowing preservation of anorectal function with lower morbidity and mortality. EID has shown effectiveness and safety in prospective studies and case report when ESD is not feasible due to technical limitations. However, further studies are required to evaluate the long-term efficacy and its comparison with other resection techniques.La disección intermuscular endoscópica (DIE) surge como una alternativa para el tratamiento de lesiones rectales con sospecha de invasión profunda o con fibrosis extensa que no permiten su resección por disección submucosa endoscópica (DSE). La DIE es una técnica que permite acceder al espacio intermuscular del recto entre las fibras circulares y longitudinales de la muscular propia. A continuación, se presenta el caso de una paciente tratada exitosamente con DIE. Paciente de sexo femenino de 54 años, con antecedente de una lesión adenomatosa sésil del recto de aproximadamente 40 mm resecada con técnica de mucosectomía convencional. Presenta una recidiva del adenoma sobre la cicatriz de resección previa. Se realiza DIE del adenoma, logrando acceder y exponer el plano intermuscular. Se reseca en bloque la lesión con márgenes histológicos laterales y profundos negativos, cumpliendo criterios de resección curativa. Las técnicas de resección endoscópicas ofrecen ventajas por sobre la proctectomía radical, preservando la función anorectal con una menor morbimortalidad. La DIE ha mostrado efectividad y seguridad en estudios prospectivos y reportes de casos donde la DSE no es técnicamente factible.
