It can metastasize through the hematogenous or lymphatic routes to different organs such as the lung and liver (common) and the stomach (uncommon). We present thecase of a 30-year-old male patient with upper gastrointestinal bleeding due to two ulcers in the stomach body with histology of unspecified small-cell malignant neoplasm. CT scans showed a right testicular tumor with secondary implants in the brain, lung, liver and spleen. β-HCG (35,5354 mIU/mL), AFP (500 ng/mL) and LDH (1,669 IU/L) tests were conducted. A right radicalorchiectomy was performed. It is concluded that this was a clinical stage IIIC primary germ cell tumor of the right testis due to liver, spleen, lung, gastric and brain metastases.Los tumores testiculares son raros; sin embargo, en la población masculina dentro del grupo etáreo de 15 a 29 años, el cáncer de testículo es el tumor sólido más común, siendo el tumor de células germinales del tipo no seminomatoso, el más frecuente. La metástasis linfática o hematógena puede darse en este tipo de tumores, siendo más frecuente que se dé a nivel pulmonar o hepático y muy infrecuente a nivel gástrico. Se presenta el caso de un paciente varón de 30 años que cursa con hemorragia digestiva alta y se realiza una gastroscopia donde se evidencia dos lesiones ulceradas en cuerpo gástrico cuyo informe de patología reporta neoplasia maligna indiferenciada de células pequeñas y redondas. Se realiza tomografía de cerebro, tórax y abdomino pélvica; donde se evidencia testículo derecho de apariencia tumoral con implante secundario en hemisferio cerebeloso izquierdo, múltiples implantes en parénquima pulmonar, en parénquima hepático, esplénico y adenopatías retroperitoneales. Asimismo, se realiza dosaje de B HCG 355354, AFP 500 y DHL 1669. Por lo tanto, se realiza orquiectomía radical derecha, con informe patológico que reporta tumor de células germinales de tipo mixto y se concluye como un tumor de células germinales primario de testículo derecho estadio clínico IIIC de alto riesgo por metástasis hepática, esplénica, pulmonar, gástrica y cerebral.Universidad de San Martín de Porres.
