Topics addressed: As discourses and approaches in GMH evolved to focus more on social determinants of mental health and materialize the Sustainable Developmental Goals, communities become increasingly important in Latin-American mental health systems. However, global crises such as the COVID-19 pandemic and climate change demonstrated that the mental health of vulnerable populations, including people with mental illness and rural and indigenous populations, were disproportionately affected, exacerbating already existing inequalities in access to appropriate care and services. On the other hand, culture is a critical factor to be considered in local responses to suffering and healing processes in the face of crises. Conclusions: There is a need to prioritize equity and inclusion in GMH care. The global and regional crises that bring significant impacts on it make the affected people and communities a valuable source of knowledge and information for the development of innovative approaches and practices that promote an equitable access to health care. Therefore, it is important to recognize their voices in formulations and approaches. Objetivos: Reflexionar sobre la evolución de los enfoques de Salud Mental Global (SMG) en los últimos quince años y su aplicación a políticas y servicios, particularmente en Latinoamérica. Describir y explicar las crisis mundiales que la han impactado y los desafíos que representan para los países de la subregión. Temas abordados: A medida que los discursos y enfoques en SMG evolucionaron para centrarse más en los determinantes sociales de la salud mental y materializar los Objetivos de Desarrollo Sostenible, las comunidades cobraron creciente importancia en los sistemas de salud mental de América Latina. Sin embargo, crisis globales como la pandemia COVID-19 y el cambio climático demostraron que la salud mental de las poblaciones vulnerables, incluidas las personas con enfermedades mentales y las poblaciones de zonas rurales e indígenas, se vieron desproporcionadamente afectadas, exacerbando las desigualdades ya existentes en el acceso a atención y servicios adecuados. Por otro lado, la cultura es un factor crítico a considerarse en las respuestas locales al sufrimiento y a procesos de sanación ante las crisis. Conclusiones: Existe la necesidad de priorizar la equidad e inclusión en el cuidado de la SMG. Las crisis globales y regionales que la afectan significativamente hacen de las personas y comunidades afectadas una valiosa fuente de conocimiento e información para el desarrollo de enfoques y prácticas innovadoras que promuevan un acceso equitativo a la atención de salud.
