Diverse pathophysiologic mechanisms are implicated in arterial hypertensionpathogenesis, including enzymatic interaction between WNK1 and SGK1 enzymes, yuxtaglomerular apparatus imbalance, angiotensin II overexcitability, pressure natriuresis, genetic factors recently studied like 11 ß-hydroxysteroid dehydrogenase enzyme gen mutation, aberrant tubular glomerular balance in relation to salt intake, and nephronal subdonor theory. Finally, the close relationship between the metabolic syndrome and arterial hypertension has opened a new horizon on arterial hypertension diagnosis and treatment. All these mechanisms have led to diverse clinical expressions, determining the concept of salt sensitive persons and the importance of low salt diet.La vida, en la evolución de las especies, en su migración del agua a la tierra, desarrolló diversos mecanismos adaptativos para la regulación de la salen un medio carente de ella. El más importante es el hoy conocido aparato yuxtaglomerular. Son varios mecanismos fisiopatológicos los implicados en la patogenia de la hipertensión arterial: interacción enzimática, entre las enzima WNK1 y SGK1, disbalance del aparato yuxtaglomerular, natriuresis por presión, sobrexcitación de la angiotensina II,factores genéticos recientemente estudiados -mutación del gen de la 11 ß-hidroxiesteroide deshidrogenasa, balance glomerulotubular aberrante en relación a la ingesta de sal y la teoría de la subdotación nefronal. Finalmente, la relación estrecha existente entre el síndrome metabólico y la hipertensión arterial ha determinado un nuevo horizonte en el diagnóstico y manejo de la hipertensión arterial.
