description Introducción. La ruptura del septum ventricular (CIV) post Infarto Agudo de Miocardio (IMA) es una complicación muy rara y grave con alta mortalidad. Reporte: paciente mujer de 79 años, con Infarto del Miocardio complicado con aneurisma ventricular y perforación del Septum interventricular (CIV). Ingresó a cirugía en NYHA IV con edema agudo pulmonar, disfunción severa del VI, fracción de eyección 30%, insuficiencia renal y obstrucción de coronaria descendente anterior del 100%. Interpretación: Se cerró el CIV con parche de Goretex y se realizó una plastía endoventricular aislando el área aneurismática con otro parche anclado en los bordes de miocardio sano.

The rupture of the ventricular septum (VSD) after acute myocardial infarction (AMI) is a rare and severe complication with high mortality. Report: 79 year old woman patient with myocardial infarction complicated with ventricular aneurysm, and perforation of the interventricular septum (VSD). He joined surgery in NYHA IV with acute pulmonary edema, severe left ventricular dysfunction, ejection fraction 30%, renal failure and anterior descending coronary artery obstruction 100%.
