Las prótesis maxilofaciales retenidas en implantes extrabucales o fijaciones cráneofaciales son herramientas claves en la rehabilitaciónde pacientes con alteraciones faciales debido a resecciones asociada con enfermedades, traumas graves, malformaciones y otros. Eluso de implantes craneofaciales se caracteriza, principalmente, por ser procedimientos quirúrgicos rápidos, efectuados con anestesiageneral o local, con poca morbilidad y gran versatilidad para tratar diferentes áreas cráneofaciales como la región orbitaria, auricular ynasal, entre otras. La reconstrucción ósea maxilar con injertos libres junto a la osteotomía Le Fort I ha sido desarrollada para dar respuesta a las necesidades clínicas funcionales y estéticas de los pacientes, debido principalmente a la discrepancia antero-posterior de las maxilas atróficas. Dentro de la técnica, existe necesidad de osteosíntesis mediante fijación interna rígida (FIR) que puede ser desarrollada tanto con dispositivos metálicos como con dispositivos reabsorbibles. El presente estudio evalúa el uso de ambos materiales de fijación, relatando las características, comparaciones y situaciones clínicas que podrían estar asociados a su elección.

Facial traumas in pediatric patients show considerations that are not present in adults, related to prevalence, diagnosis and treatment. Facial fractures prevalence in children is low, ranging from 13 to 15%. This can be due to anatomic or environmental factors, such as traffic accidents, falls and sport accidents as the main ones. The choice of treatment alternative will depend on the intrinsic characteristics of the fracture, the patient’s age and teeth development stage. Thus heterogeneity along with low incidence of facial fracture in pediatric patients, and limited experience of some clinical and surgical professionals in child trauma management can be support for lack of consensus in treatment. The aim of this study was a retrospective analysis of the incidence, characteristics, use of safety devices, treatment and complications of maxillofacial trauma caused by bicycle accidents in a pediatric and adolescent population of São Paulo. The sample was selected from analysis of 2986 medical records of victims of facial trauma patients treated entre1999 and 2008 by the Department of Oral and Maxillofacial Surgery, Piracicaba Dental School, State University of Campinas, São Paulo, Brazil. During this period, 757 patients under 18 years were treated, of which 220 were victims of bicycle accidents. The bicycle accidents constituted the main etiology (29 %) of facial trauma. Only 1.8% of patients made use of safety devices. The adolescents group and male gender were the most prevalent in all age groups evaluated.
