25 cases of endotoxic shock occurred at the Hospital between 1971 and 1980. It was presented predisposing causes endometritis in 68 percent of cases, both postpartum, cesarean section and abortion, chorioamnionitis in 20 percent hysterectomy in 2 patients and urinary infection in one. It was essentially medical treatment in 7 cases, including patients who died before being taken to the operating room. Surgical treatment consisted of dilatation and curettage in 4 patients, abdominal hysterectomy at 8, 5 exploratory laparotomy and drainage of abscess in a wall. They killed 14 women (56 percent).

It is a retrospective study of 65 cases of maternal death that occurred in the National Hospital Edgardo Rebagliati Martins (1971-1980), representing a rate of 4.0 per 10,000 live births. 63% of the mothers were 20-34 years and 37% were older than 35 years. 21% were primiparous and 17% had more than five pregnancies. 33% were full-term and 30% were less than 20 weeks pregnant. Among the causes of hospitalization they excelled: hypertensive disease 26%, febrile syndrome, abortion, premature rupture of membranes, heart failure, labor and puerperal endometritis. They were important antecedents: I abortion 24%, previous cesareans, urinary infection, heart disease, tuberculosis, stillbirths. Obstetric causes of death were infections 26% (including postpartum endometritis, infected abortion, chorioamnionitis), hipertensica disease 26%, 6% and bleeding. Clinical histories of 38 pregmant diabetic patients seen at out Hospital between 1971 and 1980 were reviewed. Thirty-two per cent were between  35 and 39 year-old and 15 per cent over 40 years old. Only 13 per cent carried their first pregnancies, and 44 per cent had 6 gestations or more. Important past history included familiar diabetes (55 per cent) and bad obstetrical history. Current pregnancy was complicated by toxemia in 18 per cent of cases and by hydramnios in 5 cases. Cesarean section was performed in 50 per cent of diabetic mothers. Twenty-one per cent of their babies were born before 37 weeks of gestational age, and other 29 per cent were macrosomic. There were five stillborn infants and one neonatal death. We believe that both perinatal mortality and hydramnios may diminish if we bring blood sugar values down to within normal limits during pregnancy.
