We present the case of a 25-year-old woman with a three-month history of cough, fever, dyspnea, pleuritic pain, and a cavitating lesion on chest tomography. She received antituberculous treatment in another health institution without a favorable response. A thoracoscopic biopsy revealed the presence of Reed Sternberg cells consistent with the nodular sclerosis subtype of Hodgkin’s lymphoma. The patient met the criteria for primary pulmonary Hodgkin’s lymphoma, an extremely infrequent and rarely reported entity. A brief review of the literature is carried out.

Objective: To determine the frequency of antibiotic use in patients with COVID-19 in a national hospital in Lima, Peru, as well as their laboratory characteristics and outcomes. Findings: 274 patients were included, with a prevalence of general and early antibiotic therapy of 45.62% and 28.1% respectively, with a mortality of 37.59%. General antibiotic therapy was associated with leukocytosis, left shift, and increased PCR. Early antibiotic therapy was associated with leukocytosis and left shift. The frequency of admission to the ICU was higher in patients with early antibiotic therapy. There was no association between general or early antibiotic therapy with mortality. Conclusions: The frequency of antibiotic use in patients with COVID-19 is high, mainly associated with leukocytosis and left shift; and does not decrease mortality.

This is the case of a 44-year-old female patient with no risk factors, extensive stroke and deep vein thrombosis. Transesophageal echocardiography showed the presence of a permeable oval foramen, which allowed a paradoxical embolism to be raised as a cause of ischemic cerebrovascular event. Otherprobable etiologies were discarded. The patient was discharged with clinical improvement receiving anticoagulation, physical therapy and rehabilitation.

Therefore, the Ministry of Health aims to achieve a standardized management and quality of perinatal maternal health; thus defined as pregnant controlled that it has complied with 6 prenatal care and which received the basic package of care to pregnant women.

We present the case of a 55-year-old male patient who was admitted due to an altered state of consciousness due to severe hyponatremia secondary to tuberculous meningitis. There was no improvement in hyponatremia after treatment with hypertonic saline solution, therefore the diagnosis of syndrome of inappropriate secretion of antidiuretic hormone (SIADH) was proposed, and improvement was evidenced with fluid restriction. The interest of this case is to report a common but forgotten complication of tuberculous meningitis.

Se presenta el caso de un paciente varón de 55 años que ingresó por alteración del estado de conciencia por una hiponatremia severa secundaria a una meningitis tuberculosa. No hubo mejoría de la hiponatremia al tratamiento con solución salina hipertónica, por lo cual se planteó el diagnóstico de síndrome de secreción inapropiada de hormona antidiurética (SIADH) y se evidenció mejoría con la restricción hídrica. El interés del presente caso es reportar una complicación frecuente pero olvidada de la meningitis tuberculosa.

Presentamos el caso de una mujer de 53 años con antecedentes de artritis reumatoide que, tras recuperarse de un cuadro de COVID-19grave, desarrolló hemorragia digestiva como manifestación de hipertensión portal e insuficiencia hepática progresiva. Basados en la serología y ecografía, se diagnosticó un síndrome overlap de hepatitis autoinmune y colangitis biliar primaria asociado a trombosis de vena porta. La respuesta a la corticoterapia y anticoagulación fue favorable. El compromiso hepático, inducido por el virus, está descrito en la COVID-19. Sin embargo, pueden desarrollarse enfermedades autoinmunes y fenómenos tromboembólicos, teniendo el hígado como órgano diana.

We present the case of a 53-year-old woman with a history of rheumatoid arthritis who, after recovering from a severe COVID-19, developed gastrointestinal bleeding as a manifestation of portal hypertension and progressive liver failure. Based on serology and ultrasound, an overlap syndrome of autoimmune hepatitis and primary biliary cholangitis associated with portal vein thrombosis were diagnosed. The response to steroids and anticoagulation was favorable. Hepatic involvement, induced by the virus, has been described in COVID-19. However, autoimmune diseases and thromboembolic phenomena can develop, having the liver as a target organ.

Introducción: La infección por el virus del dengue es una enfermedad endémica en ciertas regiones del Perú. La mayoría de estos casos se clasifican como dengue sin signos de alarma y la mortalidad reportada es menor de 1%. Sin embargo, existen ciertas condiciones asociadas a la enfermedad que podrían incrementar la mortalidad. Reporte de caso: Se presenta el caso de una paciente mujer de 48 años procedente de área endémica de esta infección con cuadro clínico y hallazgos laboratoriales compatibles con enfermedad por dengue con signos de alarma. Durante la hospitalización, cursa con hemoptisis e insuficiencia respiratoria produciendo su posterior fallecimiento. Background: Dengue virus infection is an endemic disease in some regions of Peru. Most of these cases are classified as dengue without warning signs and the reported mortality is less than 1%. However, there are certain conditions associated with the disease that could increase mortality. Case report: We present the case of a 48-year-old female patient from a dengue endemic area with clinical and laboratory compatible with dengue disease with warning signs. During hospitalization, she presents hemoptysis and respiratory insufficiency, leading to death. Conclusion: It is important to recognize diffuse alveolar hemorrhage as part of dengue respiratory manifestations and to differentiate it from other infectious and non-infectious possibilities in order to provide appropriate and early management.
