We report a case of systemic light chain amyloidosis with cardiac involvement, preceded by a monoclonal IgG lambda gammopathy. The clinical diagnosis was based on signs of heart failure, elevated biomarkers, and imaging findings. The diagnosis was confirmed by elevated light chains in the blood and urine, and the presence of amyloid deposits in the periumbilical fat biopsy and in different segments of the digestive tract. Treatment with daratumumab plus bortezomib plus dexamethasone was initiated, and autologous hematopoietic stem cell therapy was administered 22 months after diagnosis, with a favorable clinical outcome.

Objetivo. Conocer las características clínicas y determinar los factores relacionados a mayor mortalidad en pacientes con choque cardiogénico (CC) por infarto de miocardio en un hospital de referencia peruano. Materiales y métodos. Cohorte única prospectiva donde se evaluó la presentación, tratamiento y complicaciones de pacientes con CC por infarto de miocardio atendidos entre marzo 2019 a agosto 2020 en el Instituto Nacional Cardiovascular - INCOR. Se evaluaron los factores relacionados con mayor mortalidad hospitalaria y en el seguimiento, además del uso del score IABP shock II en la población de estudio. Resultados. Objetivo. Describir la calidad de vida en pacientes con arritmias ventriculares idiopáticas, tratados en el Instituto Nacional Cardiovascular INCOR de Lima-Perú. Materiales y métodos. Estudio analítico y transversal de pacientes con arritmias ventriculares idiopáticas, tratados mediante ablación 3D o terapia antiarrítmica entre julio 2017 y diciembre de 2019 a quienes se aplicó el cuestionario de salud SF-36 para evaluar la calidad de vida relacionada con la salud. Resultados. Se incluyeron a 52 pacientes con arritmias ventriculares idiopáticas (34 fueron a ablación 3D y 18 solo terapia antiarrítmica). Objetivos: Se desea saber la incidencia, los factores asociados y la mortalidad a 30 días de los pacientes con insuficiencia cardiaca (IC) postinfarto de miocardio con elevación del segmento ST (IMCEST) en Perú. Métodos: Estudio observacional, de cohortes, multicéntrico a nivel nacional, de pacientes enrolados en el registro peruano de IMCEST, excluyendo los pacientes con antecedente de IC. Se compararon las características epidemiológicas, tratamiento y evolución a 30 días de los pacientes con (grupo 1) y sin (grupo 2) IC postinfarto. Resultados: De 388 pacientes se encontró un 48.7% con síntomas de IC o fracción de eyección de ventrículo izquierdo < 40% postinfarto (grupo 1). Objectives: The aim of this study is to determine the incidence, associated factors, and 30-day mortality of patients with heart failure (HF) after ST elevation myocardial infarction (STEMI) in Peru. Methods: Observational, cohort, multicentre study was conducted at the national level on patients enrolled in the Peruvian registry of STEMI, excluding patients with a history of HF. A comparison was made with the epidemiological characteristics, treatment, and 30 day-outcome of patients with (Group 1) and without (Group 2) heart failure after infarction. Results: Of the 388 patients studied, 48.7% had symptoms of HF, or a left ventricular ejection fraction <40% afterinfarction (Group 1). A retrospective descriptive study was carried out. During February 2020, data was collected from the medical records of all patients in whom ablation was performed using 3D mapping from July 2017 to December 2019. This procedure was performed in patients with symptomatic arrhythmia refractory to antiarrhythmic therapy. Results. Objetivo. Describir la experiencia inicial en ablación de arritmias cardiacas empleando mapeo 3D en el Instituto Nacional Cardiovascular INCOR (Lima-Perú). Materiales y métodos. Estudio descriptivo, retrospectivo, donde se recolectaron datos de historias clínicas de todos los pacientes en los cuales se realizó ablación empleando mapeo 3D, desde julio de 2017 a diciembre de 2019.
