description Objetivo: evaluar la evidencia científica sobre la eficacia, seguridad y limitaciones de los tratamientos de farmacoterapia moderna en adultos mayores con neumonía nosocomial. Materiales y métodos: se realizó una revisión sistemática en PubMed, EBSCO Host y RedALyC, utilizando descriptores en inglés y español junto con operadores booleanos. Se aplicaron filtros para los últimos cinco años, excluyendo metaanálisis, revisiones, protocolos y documentos no originales. De 979 registros identificados, tras aplicar criterios de inclusión, exclusión y eliminar duplicados, se seleccionaron 12 estudios para análisis crítico. Resultados: los estudios evidencian que nuevos antibióticos, como sulbactam–durlobactam e imipenem/cilastatina/relebactam, son eficaces y mejor tolerados en adultos mayores con neumonía nosocomial. La colistina inhalada combinada con meropenem aumenta la eficacia y reduce la toxicidad frente a la vía intravenosa. Asimismo, esquemas cortos (3–7 días) resultan igual de efectivos que los prolongados, con menor riesgo de resistencia antimicrobiana. Conclusión: nuevos antibióticos y estrategias como la colistina inhalada y la individualización del tratamiento mejoran los resultados clínicos y reducen eventos adversos. Estos enfoques favorecen el uso racional de antimicrobianos, optimizan la atención en pacientes frágiles y representan un avance hacia una terapia más sostenible frente a la resistencia bacteriana.

Materials and methods: a systematic review was conducted using the PubMed, EBSCO Host, and RedALyC databases, with English and Spanish descriptors combined with Boolean operators to optimize the search. Filters were applied to restrict the results to the last five years, excluding meta-analyses, reviews, protocols, and non-original documents. A total of 979 records were identified; after applying inclusion and exclusion criteria and removing duplicates, 12 studies were selected for critical analysis. Results: the reviewed studies show that new antibiotics such as sulbactam–durlobactam and imipenem/cilastatin/relebactam are effective and better tolerated in older adults with nosocomial pneumonia. Inhaled colistin combined with meropenem improves efficacy and reduces toxicity compared to intravenous administration. Additionally, short treatment courses (3–7 days) are as effective as prolonged regimens, with a lower risk of antimicrobial resistance. Conclusion: new antibiotics and strategies, including inhaled colistin and individualized treatment duration, have shown improved clinical outcomes and reduced adverse events.
