Fetal pharmacologic therapy can be defined as any prenatal treatment administered to the mother with the primary indication of improving perinatal or long-term outcomes for the fetus and the newborn. This review provides an update on the pharmacological therapies for the fetus with anomalies. Fetal anomalies targeted with prenatal pharmacotherapy are a heterogeneous group of structural, endocrine, and metabolic conditions, including brain and spine anomaly, hyaline membrane disease prevention, congenital cystic adenomatoid malformation (CCAM), congenital adrenal hyperplasia, congenital heart block, fetal tachyarrhythmia, inborn errors of metabolism, fetal thyroid disorders, and polyhydramnios. To date, most of the pharmacotherapy for fetal anomalies has been evaluated only in retrospective, uncontrolled studies. Future evaluation will be determined on evidence-based approach to prenatal ...

Objective. To report a new approach for perinatal and pediatric necropsy that allows parental consent. Methods. Experience with necropsies in 20 stillborn, 15 neonates and 8 infants is presented with this new approach; the parents had refused the use of the standard body incision. Through a small transversal about 10 cm incision in the suprapubic region, manual evaluation of the abdominal and pelvic cavities was performed, and then the viscera were obtained by either the Letulle`s technique (a single block) or the Virchow´s technique (organ by organ). The incision is closed by subdermic suture. Conclusions. This technique is well accepted by the parents because the skin surface and the body as a whole appear little disturbed.

Caesarean section is performed in 30% of births and is the most common surgical procedure in the United States. In Peru, the cesarean section rate has remained around 21% in the public sector, in the private sector rates have accelerated since the beginning of this decade, going from 28.6% in the period 1991-1993 to 48, 6% in the period 2002-2005. In the 2002-2005 period, the cesarean section rate was 43% for women without insurance and 69% for women who had private health insurance. Regional anesthesia techniques are currently the most used for cesarean section, and are used even in situations that were previously considered an indication for general anesthesia (cord prolapse, preeclampsia, placenta previa). Although the caesarean section rate has remained constant (23.1-30%), the use of general anesthesia has greatly decreased (0.8% of all caesarean sections). Arterial hypotension is t...

The intrauterine fetal growth retardation (IUGR) represents 1 to 7% of all deliveries in countries like ours and ¡s caused by malnutrition, pregnancy induced hypertension, poverty, low pregestacional weight, short intergenesic intervals, maternal complications. Preterm delivery and perinatal morbidity and mortality are more frequent. Decreased levels of interleukin 3, stimulant growth factors, receptors of epidermal growth factor, and increase of catecholamines, corticoids, and other fetal steroids have been found. The latter three increase to improve the hypoxemic circulation, maintain glucose delivery to heart and brain and for lung maturation. Placenta in IUGR shows thinning, infarction and decreased levels of angiotensine II, atrial natriuretic peptide, glucose, HPL, estriol, progesterone, and receptors of growth factors. Immune feto placental system ¡s affected, with presence of c...

AND “treat” AND NOT TITLE "Systematic review" AND NOT TITLE “review”. Of Maternal-Fetal And Neonatal Me...

Iron is essential for health but in excess may be harmful. The living organism has ability to store and re-use the iron body and as such its requirement is small (1-2 mg absorbed/day). Hepcidin, a hormone produced in the liver, has an important role in iron homeostasis by blocking the iron transport protein and inhibiting iron absorption. Maternal anemia due to iron deficiency is a public health problem when it is moderate (7-<9 g/dL) and severe (Hb<7 g/dL) since it increases the risk of maternal and neonatal morbi-mortality. Similarly, high levels of hemoglobin during pregnancy affect the mother and newborn. In Peru, there is a National Program to supplement with iron to all pregnant women anemic or not. Scientific evidence shows that hemoglobin values between 9-10.5 g/dL are optimal for best fetal growth and that treatment with iron supplementation to non anemic women increases levels ...
