Objetivos: Determinar la asociación entre tratamiento previo antituberculoso (antiTB) y el desarrollo de drogorresistencia a los fármacos antituberculosos en pacientes mayores de 18 años con diagnóstico de tuberculosis (TB) pulmonar. Métodos: Estudio de casos y controles. Los pacientes tuvieron prueba de sensibilidad rápida y recibieron tratamiento antiTB en el Puesto de Salud Trébol Azul, Lima, Perú, entre 2010 y 2015. La drogorresistencia fue clasificada como Monorresistencia a Isoniazida o Rifampicina y Multidrogorresistencia. Se empleó el modelo de regresión logística y odds ratio (OR), con intervalo de confianza al 95% (IC 95%). Resultados: Evaluamos 129 pacientes (58 casos y 71 controles). 44 (34%) de pacientes tuvieron tratamiento antiTB previo. Las prevalencias de resistencia a isoniazida, rifampicina y multidrogorresistencia fueron 12%, 4%, y 29%, respectivamente. En el análisis multivariado los pacientes con tratamiento antiTB previo tuvieron 14 veces más riesgo de desarrollar drogorresistencia en comparación a aquellos sin tratamiento previo (OR ajustado 14,42; IC 95% 4,40-47,21; p<0,001). Conclusiones: En este estudio unicéntrico el antecedente de tratamiento previo antiTB estuvo asociado a 14 veces más riesgo de presentación de drogorresistencia. Ningún otro potencial factor de riesgo definido a priori estuvo asociado con mayor riesgo de drogorresistencia. |

Methods: Case and control study. The patients had a rapid sensitivity test and received anti-TB treatment at the Trébol Azul Health Post, Lima, Peru, between 2010 and 2015. The drug resistance was classified as Isoniazid Monoresistance or Rifampicin and Multidrug Resistance. The logistic regression model and odds ratio (OR) were used, with a 95% confidence interval (95% CI). Results: We evaluated 129 patients (58 cases and 71 controls). 44 (34%) of patients had previous anti-TB treatment. The prevalences of resistance to isoniazid, rifampicin and multidrug resistance were 12%, 4%, and 29%, respectively. In the multivariate analysis, patients with previous anti-TB treatment had a 14-fold greater risk of developing drug-resistance compared to those without prior treatment (adjusted OR 14.42, 95% CI 4.40-47.21, p <0.001). Conclusions: In this single-center study, the history of previous anti-TB treatment was associated with a 14-fold increased risk of drug resistance.
