description Determinar si la fragilidad es un predictor de morbilidad y mortalidad luego de una colecistectomía en pacientes adultos por colecistitis aguda. Materiales y Métodos: Se realizó un estudio de casos y controles anidados en una cohorte en el Hospital Regional Docente de Trujillo durante el periodo comprendido entre enero del 2019 y diciembre del 2022; la muestra estudiada fue de 86 pacientes adultos mayores con diagnóstico de colecistitis aguda a quienes se les realizó colecistectomía. Resultados: El análisis bivariado encontró que el score ASA categoría III (33,33% vs 5,41%; p = 0,002), la estancia hospitalaria (11,08 ± 6,14 vs 3,11 ± 1,85 días; p = 0,001) y la fragilidad (75% vs 37,84%, p = 0,016) estuvieron asociadas a la presencia de complicaciones. El análisis multivariado para predecir complicaciones en pacientes con colecistitis aguda postoperados de colecistectomía en adultos mayores, identificó como variables independientemente asociadas al score ASA (ORa = 15,26 IC 95% [2,27 – 102,41]) y la fragilidad (ORa = 7,60 IC 95% [1,45 – 39,73]). Conclusiones: La fragilidad si es un factor asociado a la presencia de complicaciones y debería ser considerada en el preoperatorio de los pacientes con colecistitis aguda que serán sometidos a colecistectomía
