Descripción del caso: Varón de 49 años, sin comorbilidades ni antecedente de infección tuberculosa, que inició dolor lumbar irradiado a zonas inferiores, posteriormente el dolor se establece en cadera izquierda con gran incapacidad funcional. Los estudios de imagen y de laboratorio confirmaron la sospecha de tuberculosis coxofemoral, con una colección purulenta articular, iniciando terapia antifímica y un drenaje quirúrgico exitoso. Conclusiones: La TBC de cadera puede presentarse de manera aislada, sin manifestaciones sistémicas, en un sujeto inmunocompetente, debiendo tenerse en cuenta como un diagnóstico diferencial ante dolor de cadera.
