Tools such as the modified early warning score (MEWS) and the rapid COVID-19 severity index (qCSI) have proven to be useful in the identification of patients with severe COVID-19, facilitating their monitoring and allowing timely management in intensive care units, which could improve the quality of care and reduce mortality. Objective: To determine whether the MEWS scale is more valid than the qCSI scale for predicting in-hospital mortality in SARS patients with severe COV-2. Methodology: An analytical cross- sectional study was carried out in the database of the ICU service of the Hospital Regional Docente de Trujillo, on 310 patients with a diagnosis of severe COVID- 19 during the years 2020 to 2023. The IBM® SPSS® Statistics Version 25.0 statistical program was used to process the data obtained. A bivariate and multivariate analysis was performed by calculating the crude and adjusted prevalence ratio (PR) between all those factors of exposure and mortality in patients with severe COVID-19. Results: When evaluating mortality with respect to the scales, the prevalence of high risk according to MEWS was different in deceased and survivors (p<0.001); however, no difference was found with the qCSI scale (p=0.051). A multivariate analysis was performed, adjusted for age, days of hospitalization, urea, sex and asthma, where it was found that the RPa of MEWS Score was 1.38 (95% CI: 1.16-1.66, p<0.001) and the RPa of qCSI SCORE was 1.39 (95% CI: 1.06-1.85, p<0.05).
