Se revisaron historias clínicas y se recolectaron datos sobre tipo de cirugía, absceso residual, edad, sexo, diabetes mellitus tipo 2, leucocitosis, índice neutrófilo/linfocito y tiempo operatorio. El análisis se realizó en Stata 17, utilizando Chi cuadrado para variables categóricas, prueba U de Mann-Whitney para no paramétricas, y regresión logística cruda y ajustada, considerando significativo un p<0.05. Se contó con autorización institucional. RESULTADOS: La incidencia de absceso residual fue 3.5 % en cirugía abierta y 2.5 % en laparoscópica. En ambos tipos de cirugía leucocitosis, diabetes mellitus y tiempo operatorio elevado se mostraron asociados. Tras ajuste en regresión logística, en cirugía abierta persistieron como factores independientes la leucocitosis y el tiempo quirúrgico prolongado, mientras que en laparoscópica fueron la diabetes mellitus tipo 2 y el tiempo quirúrgico prolongado. CONCLUSIONES: La apendicectomía abierta presentó mayor incidencia de absceso residual, aunque sin significancia estadística. Factores como leucocitosis, diabetes y tiempo quirúrgico prolongado incrementan el riesgo de absceso residual, destacando la necesidad de optimizar los tiempos operatorios para reducir complicaciones postoperatorias

METHODS: Observational, analytical, retrospective cohort study including 400 patients over 18 years old with uncomplicated acute appendicitis (200 underwent open surgery and 200 laparoscopic surgery), treated between 2022 and 2024. Medicales records were reviewed, collecting data on type of surgery, residual abscess, age, sex, type 2 diabetes mellitus, leukocytosis, neutrophil-to-lymphoc yte ratio, and operative time. Data analysis was performed in Stata 17, using Chi-square test for categorical variables, Mann-Whitney U test for non-parametric variables, and both crude and adjusted logistic regression, considering p<0.05 significant. Institutional authorization was obtained. RESULTS: The incidence of residual abscess was 3.5% in open surgery and 2.5% in laparoscopic surgery. In open surgery, leukocytosis, type 2 diabetes mellitus, and prolonged operative time were associated with a higher risk of residual abscess. In laparoscopic surgery, leukocytosis, diabetes, and prolonged operative time also showed association. After logistic regression adjustment, leukocytosis and prolonged operative time remained independent factors in open surgery, while in laparoscopic surgery, type 2 diabetes mellitus and prolonged operative time were independent predictors. CONCLUSIONS: Open appendectomy showed a higher incidence of residual abscess, although not statistically significant.
