description Objetivo: Determinar la asociación de los desenlaces clínicos histerectomía y prematuridad en pacientes gestantes con placenta previa atendidas en el Hospital III-1 José Cayetano Heredia – Piura, entre el periodo de 2014 a 2024. Material y métodos: Se realizó un estudio observacional, analítico y retrospectivo de cohorte. Se incluyeron 177 gestantes con diagnóstico ecográfico de placenta previa en tercer trimestre, clasificadas según tipo de inserción placentaria. Se evaluaron desenlaces maternos y perinatales, con énfasis en la prematuridad y la histerectomía. Se utilizó estadística descriptiva, análisis bivariado y regresión para estimar riesgos relativos (RR) crudos y ajustados con intervalos de confianza al 95%, usando el programa Stata v15. Resultados: La placenta previa total fue la más frecuente (60.5%) y se asoció significativamente con parto prematuro (RRa: 1.51; IC95%: 1.07–2.14) y hemorragia postparto (RRa: 4.89; IC95%: 1.26–18.69). Se evidenció una tendencia a un aumento del riesgo de histerectomía (RRa: 2.16; IC95%: 0.84– 5.56). Conclusiones: La placenta previa total incrementa el riesgo de prematuridad, histerectomía y hemorragia postparto, lo que justifica un manejo obstétrico especializado.

Material and methods: An observational, analytical, retrospective cohort study was conducted. A total of 177 pregnant women with a third-trimester ultrasound diagnosis of placenta previa were included and classified according to placental insertion type. Maternal and perinatal outcomes were evaluated, focusing on prematurity and hysterectomy. Descriptive statistics, bivariate analysis, and regression modeling were used to estimate crude and adjusted relative risks (RR) with 95% confidence intervals, using Stata v15. Results: Total placenta previa was the most frequent type (60.5%) and was significantly associated with preterm delivery (aRR: 1.51; 95% CI: 1.07–2.14) and postpartum hemorrhage (aRR: 4.89; 95% CI: 1.26–18.69). A trend toward an increased risk of hysterectomy was observed (aRR: 2.16; 95% CI: 0.84–5.56).
