El estudio tuvo como finalidad determinar los factores clínicos, quirúrgicos y postoperatorios asociados a la mortalidad hospitalaria en pacientes intervenidos por obstrucción intestinal en el Hospital Regional Cayetano Heredia – EsSalud, Piura, durante el periodo 2021–2025. Se desarrollo un estudio observacional, analítico, de tipo casos y controles, que incluyó 133 pacientes postoperados, divididos en 66 fallecidos (casos) y 67 sobrevivientes (controles). Se analizaron variables sociodemográficas (edad, sexo), clínicas (hipoalbuminemia, lactato sérico), intraoperatorias (tipo de procedimiento, tiempo operatorio, reintervención), postoperatorias (complicaciones infecciosas y sistémicas) y de estancia hospitalaria. Los datos se procesaron mediante análisis bivariado y regresión logística multivariada para identificar factores independientes de mortalidad. Los resultados mostraron que la reintervención quirúrgica (OR = 3,0.1; p = 0,037). Entre las complicaciones postoperatorias, la insuficiencia respiratoria aguda, sepsis y shock séptico fueron los predictores independientes en el modelo ajustado (p < 0,05). Se concluye que las complicaciones postoperatorias graves constituyen los factores determinantes de mortalidad hospitalaria, resaltando la necesidad de una vigilancia intensiva para un manejo temprano de sepsis y falla respiratoria para reducir los desenlaces fatales. |

The present study aimed to identify the clinical, surgical, and postoperative factors associated with in-hospital mortality in patients operated on for this condition at the Hospital Regional Cayetano Heredia – ESSALUD, Piura, between 2021 and 2025. An observational, analytical, case-control study was conducted, including 133 adult patients who underwent emergency surgery for intestinal obstruction. Two groups were formed: 66 patients who died (cases) and 67 who were discharged alive (controls). Preoperative clinical factors were considered, such as advanced age, serum albumin and lactate levels, symptom duration, and the presence of acid–base disorders; as well as the etiologies of mechanical intestinal obstruction (hernia, adhesions, tumors, volvulus, eventration), surgical technique used, operative time, and intraoperative findings (ischemia, necrosis, perforation, hemorrhage, adhesions, torsion, peritonitis, and thrombosis). The type of surgical procedure performed (resection and anastomosis, colostomy, ileostomy, surgical reintervention), postoperative complications (sepsis, hospital-acquired pneumonia, septic shock), and hospital stay were also analyzed. Statistical analysis employed Chi-square, Fisher’s exact, Mann–Whitney U tests, and binary logistic regression to identify independent predictors. The results showed that surgical reintervention remained an independent predictor, tripling the risk of mortality (adjusted OR: 3.01; 95% CI: 1.07–8.47; p = 0.037). In addition, three postoperative complications were identified as independent predictors of mortality: acute respiratory failure, which increased the risk of death nearly fourteenfold (adjusted OR: 13.8; 95% CI: 1.8–105.9; p = 0.011); sepsis, which raised the risk approximately thirty-twofold (adjusted OR: 31.9; 95% CI: 2.97–343.4; p = 0.004); and septic shock, associated with a twenty-fivefold higher probability of death (adjusted OR: 24.7; 95% CI: 2.2–278.4; p = 0.010).
