Sumario:Formulation of the problem: What is the impact of home clinical follow-up and the perception of the quality of life of patients with type 2 diabetes mellitus, who were discharged from the medical service of the regional hospital of Loreto, from July 2021 to July 2022?, General objective: To evaluate the impact of home clinical follow-up and the perception of quality of life of patients with type 2 diabetes mellitus who were discharged from the medical service of the regional hospital of Loreto, from July 2021 to July 2022; Specific Objectives: To identify the sociodemographic and clinical characteristics of patients with Type 2 Diabetes Mellitus who were discharged from the medical service of the regional hospital of Loreto, from July 2021 to July 2022, To describe the satisfaction of the treatment of the disease and the concern of the future effects of the disease of patients with type 2 diabetes mellitus who were discharged from the hospital medicine service Loreto Regional, July 2021 to July 2022, Demonstrate the impact of permanent home clinical evaluations on reducing hospital readmissions of patients with type 2 diabetes mellitus who were discharged from the medical service of the Loreto Regional Hospital, from July 2021 to July 2022, To demonstrate that lifestyle changes, with special emphasis on appropriate eating habits and regular physical exercise, reduce decompensation in patients with type 2 diabetes mellitus who were discharged from the medical service of the Loreto Regional Hospital, from July 2021 to July 2022, Describe the perception of quality of life of patients with type 2 diabetes mellitus who were discharged from the medical service of the regional hospital of Loreto, from July 2021 to July 2022. Main hypothesis: There is an important impact of home clinical follow-up and a regular perception of the quality of life of patients who were discharged from the HRL medicine service. Null hypothesis: There is NO significant impact of home clinical follow-up and a regular perception of the quality of life of the patients who were discharged from the HRL medicine service. Sample Design, Study Population: It will be made up of all diabetic patients admitted and discharged from the HRL medicine service from July 2021 to July 2022. Sampling: the Simple Probabilistic Random Sampling formula will be used and yields a minimum sample of 20 patients. The sampling will be simple probabilistic, the medical records of hospitalized patients from July 2021 to July 2022 will be analyzed. Selection criteria, Inclusion Criteria: Patients older than 18 to 65 years of age with diabetes discharged from the medical service from July 2021 to July 2022. Exclusion Criteria: diabetic patients over 18 to 65 years old graduated from the medical service who do not wish to participate in the study or who are disabled due to some cognitive or motor condition. Instrument techniques or data collection, Instrument: The Physician will request authorization from the directors of the Loreto Regional Hospital for the execution of this project. Processing and analysis: For patients who meet the inclusion criteria, a corresponding review of the database will be made. The results will be analyzed using descriptive statistics with measures of central tendency and dispersion. The results will be analyzed using descriptive statistics with the T Students test and chi square. The information collected will be entered into a spreadsheet of the statistical package SPSS version 22.0. Descriptive statistics will be used for the presentation of frequency tables and percentages. Ethical aspects: Permission to carry out the study will be requested from the postgraduate of the Faculty of Medicine and the ethics committee of the hospital. The patient's authorization will be requested through informed consent, handling the information collected confidentially. BUDGET: S/ 4350 Soles
