La búsqueda se efectuó en PubMed y Scopus, incluyendo estudios clínicos y farmacogenéticos en población adulta hipertensa tratada con IECA, que evaluaron la relación entre polimorfismos genéticos y respuesta terapéutica. Resultados: se analizaron 12 estudios internacionales, con tamaños muestrales entre 45 y más de 9 000 participantes. Cinco investigaciones reportaron asociaciones positivas o parcialmente favorables entre determinadas variantes genéticas y la eficacia o seguridad de los IECA. Cinco estudios no evidenciaron diferencias significativas en la reducción de la presión arterial según el perfil genético, mientras que dos mostraron efectos condicionados a desenlaces específicos, como sexo o eventos adversos. Conclusión: la evidencia indica que la eficacia antihipertensiva de los IECA no depende de manera consistente de variantes genéticas aisladas. La farmacogenética podría ser útil en contextos específicos, principalmente para seguridad o subgrupos particulares, más que para la respuesta global de presión arterial.

Materials and methods: a systematic review was conducted following PRISMA guidelines. The search was performed in PubMed and Scopus, including clinical and pharmacogenetic studies in adult hypertensive patients treated with ACE inhibitors that evaluated the relationship between genetic polymorphisms and therapeutic response. Results: twelve international studies were analyzed, with sample sizes ranging from 45 to more than 9,000 participants. Five studies reported positive or partially favorable associations between specific genetic variants and the efficacy or safety of ACE inhibitors. Five studies found no significant differences in blood pressure reduction according to genetic profile, while two showed effects conditioned to specific outcomes, such as sex or adverse events. Conclusion: the available evidence indicates that the antihypertensive efficacy of ACE inhibitors does not consistently depend on isolated genetic variants.
