La búsqueda se efectuó en PubMed, Scopus, ScienceDirect, Web of Science y Epistemonikos, incluyendo estudios publicados entre 2020 y 2025. Se seleccionaron ensayos clínicos y estudios observacionales comparativos en adultos con fibrilación auricular. Se excluyeron revisiones, metaanálisis, reportes de casos y preprints. Los datos se extrajeron mediante una matriz estandarizada y se sintetizaron narrativamente. Resultados: la mayoría de los estudios mostró que los DOAC tienen una efectividad comparable o superior a warfarina en la reducción de accidente cerebrovascular y embolia sistémica, con un mejor perfil de seguridad y menor riesgo de hemorragia mayor. Estos beneficios se mantuvieron en subgrupos de alto riesgo como adultos mayores. Conclusión: los DOAC son una alternativa eficaz y generalmente más segura que warfarina en adultos con fibrilación auricular, con un beneficio clínico neto positivo.

Materials and Methods: a systematic review was conducted according to PRISMA 2020 guidelines. The search was performed in PubMed, Scopus, ScienceDirect, Web of Science, and Epistemonikos, including studies published between 2020 and 2025. Comparative clinical trials and observational studies in adults with atrial fibrillation were selected. Reviews, meta- analyses, case reports, and preprints were excluded. Data were extracted using a standardized matrix and synthesized narratively. Results: most studies showed that DOACs have comparable or superior effectiveness to warfarin in reducing stroke and systemic embolism, with a better safety profile and a lower risk of major bleeding. These benefits were maintained in high-risk subgroups, such as older adults.
